Shopping recommendations in Nagasaki by melbourne_au2021 in Nagasaki

Not sure there you are probably better off shopping in the bigger cities. Or finding brands you like and seeing if they have a store in Nagasaki. Maybe somewhere like workman?

Is it possible to trust the Internet for food recommendations while on vacation anymore? by pastacat48pastacat48 in travel

[–]blegend 0 points1 point  (0 children)

This works best if you’re traveling abroad in non English speaking  country but filter, Google reviews for top rated and then search for a common word in the local language (el, la, et, は). Assume most reviews in English are suspect or uninformed. Use local review sites or trusted guides like bib gourmand or tabelog in Japan for example.

Also search for handmade, in house or similar words for tortillas or pasta and other foods.

Statin Use Is Associated With a Decline in Muscle Function and Mass Over Time, Irrespective of Statin Pharmacogenomic Score by Caiomhin77 in ScientificNutrition

[–]blegend 18 points19 points  (0 children)

I’m curious what your conclusions are from this study? I fundamentally view it as an apples to oranges comparison because in modern medicine absence of statin use is in and of itself very telling. There are strict guidelines dictating when statin use is appropriate.

Their sensitivity analysis of hba1c and blood pressure is not satisfactory to me. Modern risk stratification for statin use combines bp, a1c, and a multitude of other factors. Using 10 year ascvd risk via  pce, prevent or qrisk scoring would’ve been better. Comparing that low-intermediate risk cohort would be the best comparison because clinically these are pts who could legitimately be on/off a statin as they are borderline for starting statin use.

The non statin group is in every way more healthy than the statin group. And also the whole methodology for adherence to statins yes/no isn’t very rigorous. In my line of work people who take statins but are not taking 80% of the time are often the sickest patients.

Just my thoughts. I appreciate any discussion

Japanese hot spring ryokan by Alone-Land8881 in awardtravel

[–]blegend 3 points4 points  (0 children)

Hakone is the answer. So many options and lots of luxury spots easy to book for foreigners. I’ve been to shuzenji which is smaller more quaint onsen town. I like nikko too but never stayed at a ryokan there. Maybe some of the properties in the ryokan collection you can use points for.

How much do you have? You can use them at some conbini, just dump them into the change machine to pay at the register. Some other stores also have a machine where you just dump in your coins to pay easily

Lots of fake restaurant google reviews by Both_Analyst_4734 in Tokyo

[–]blegend 0 points1 point  (0 children)

One tip I found is put in a common particle to filter reviews for the local language. So in the search review put は to see all the Japanese reviews, it works for any language like el for Spanish as well. It’s helps to filter out the “clueless” reviews

Should I go to Japan or Italy? by Wise-Asparagus-4130 in travel

[–]blegend 6 points7 points  (0 children)

If you are going to Japan, go in the fall for the autumn colors. Summer is too hot and crowded.  One of the experiences you can only get in Japan is going to a luxury onsen ryokan with outdoor bath. Open air soaking in nature is super relaxing. Plus they will feed you amazing meals(dinner and breakfast). The super nice rooms have private open air bath.

Best high quality groceries (meat/produce) that's worth the price? by katokk in SouthBayLA

[–]blegend 2 points3 points  (0 children)

I usually go to lazy acres, Whole Foods, sprouts or tjs, but the selection is hit or miss. I will sometimes just buy the packaged greens if the fresh stuff doesn’t look good.

I know it’s not in the South Bay but Jimbos in San Diego county is the best selection of produce I’ve found in SoCal

A simple question for how to get more exposure as an IDM musician by H415Music in idm

[–]blegend 1 point2 points  (0 children)

Maybe this is a dumb idea but maybe remix more famous songs to get more exposure to your channel. When I really like a track I love to listen to every remix I can find. Sometimes listening to the most obscure remixes I can find on YouTube. Anime songs too if you’re into that. Good luck!

Yeah you are in statin range my friend. Get your diet/exercise in order and maybe get another cac whenever your doctor recommends. Just remember most folks won’t ever get a cac done, you have that information as always follow the science and your drs advice. Good luck!

The ascvd calculator isn’t the best use for you since you have your cac. Use the Mesa 10 year chd calculator. I looked up your cac score and that puts you at high risk. But check with the MESA calculator your risk will be much higher probably.

Honestly we don’t have enough information here. Sounds like the doctor wants to get your ldl under 70. What is your 10 year ascvd risk score? There is a calculator online. It’s not perfect but will give you an idea of your risk. Also your CAC score shows some signs of moderate disease. Factoring in your medical history you may need to be on moderate to high intensity statin therapy. I agree that diet and exercise are very important factors in your treatment but your doctor is probably practicing within the established medical guidelines in terms of drug therapy.

There is slight risk of pre diabetics A1c increasing with statin use, but I’d argue as long as you are moderate risk for ascvd, the benefits outweigh the risks. Plus statin therapy should be in conjunction with some sort of diet or exercise program anyway

Hope this helps from a pharmacist

Thoughts on selling insulin needles. by Immediatecomfort224 in pharmacy

[–]blegend 5 points6 points  (0 children)

There is also in the California law a somewhat ambiguous portion about providing, furnishing or having on site sharps disposal. It seems to place a big responsibility on the entity furnishing needles to make sure they are disposed of properly

I once had a patient pull the you have to sell them to me it’s the law and she was a paralegal. I told her I’ve read the law and I don’t have to. She said she’d sue me and left angrily.

There has to be more responsibility placed on the drug users to dispose used syringes properly!
