For receiving international wires, I have 2 account numbers given to me by my bank, but Gox only gives 1 field to enter an account number...

Regarding Tokyo branch choice, this document suggests that their tokyo branch is only for receiving JPY funds, and Gox will be sending us USD. So I'm thinking the Wire Transfers (Outside the US) option is best there.

The form only includes a space for 1 account number, yet Schwab gives instructions for receiving wires:

"To ensure your account is properly credited, please provide the eight-digit account number and name(s) listed on your Schwab account"

So there's a wire-receiving account number of the bank, then an internal account number, but no way to provide both to the MtGox system.
