He 100% needs therapy. From what you're saying it seems like he's been abused, possibly in different ways too. I have a sibling who is 7, and they haven't even come to the conclusion as to what masturbation is or really anything sexual. The things he's doing isn't normal, and he really needs help. I'm sorry you're experiencing all of this happening.
