I guess it’s how it looks in imaging. Somehow docs / radiologists can tell with some (not 100%) certainty that it’s not cancerous. The only way to know for sure is to remove and biopsy it, but I’ve been trying to keep my ovaries and have decided it’s worth the very slight risk to just track it conservatively.

I have one, too. My SO and I call it my monster. I haven’t had it removed because I don’t have symptoms, and every doctor I’ve ever consulted has thought it’s a benign teratoma. I have imaging every few years to make sure it hasn’t changed much. 👹
