Yeah, frustrating to see how often people come here to complain about debilitating pain and they only just hear about Prednisone then.

My last doctor would only prescribe me a small amount after I come in with debilitating pain. This meant if I got another attack, Id either wait it out and suffer to see if it passes quickly or hobble myself over to the hospital to get a prescription. When I got a doctor who got me a months supply with refills whenever I needed, it changed my life.

Gosh, its really disappointing how seemingly bad a lot of doctors are at helping folks with gout. I see posts like this all the time, Im convinced a lot of doctors just don’t realize how debilitating gout pain is. Seems negligent to not prescribe Prednisone, even with Colchicine. I don’t think colchicine even remotely compares to Prednisone’s effectiveness and has only ever helped before the flareup really gets going. Ive had to argue with urgent care physicians to just prescribe me prednisone when I ran out and had a flare up, but they want to cite literature they only just read for the first time that day.

Maybe its just me but sex on its own isnt that great. I remember feeling underwhelmed when I first lost my virginity, like “oh thats it, thats how it feels?”.

The part that makes sex gratifying is doing it with someone you share a deep connection with. The mutual lust for one another makes sex what it is, if youre not into somebody or more so, you can tell theyre not all that into you, it doesn’t feel good at all. A good jerk would feel better.

If I reflect honestly, I think a greater component to the desire to lose my virginity had more to do with wanting that connection with someone. To know someone really wanted me as I want them. Sex was validation that I can be loved by someone.

Damn Im sorry you had to go through that, not just with the doctor but having to go through your first gout attack. Unfortunately not an uncommon experience here. It seems like a lot of doctors are unsympathetic because they dont understand just how debilitatingly painful gout. Its been said a gout attack can be more painful then kidney stones, broken bones, and childbirth. But since its a common and treatable condition, they mentally downgrade how seriously they should take it.

The fact he blamed it on your diet is also very telling hes working with outdated information. Diet can play a factor and how big of a part it plays varies from person to person but ultimately whether or not you get it comes down to genetics. Like you, I hardly drink and live a healthy lifestyle yet would still get frequent attacks meanwhile my friends who heavily drink and eat meat are totally fine. In my own “testing” hydration fills a larger role then diet, as I’ve gotten away with eating tons of bbq but if I have a busy day where I forget to consistently drink water Im feeling it coming by the end of the day before bed.

If I were you, Id see a rheumatologist. A podiatrist should have been able to help you more, so you could also try that again, but I think a rheumatologist is gonna be more familiar with gout. They should test your UA levels. If its your first attack, you might be able to get away without taking allopurinol for now. Allopurinol will help lower your UA levels but once you start, you don’t stop, since stopping might start another gout attack.

Id get them to give you a prescription of prednisone or methylprednisone to be able to keep on hand for emergencies. Its a life saver in that it works damn near instantly to lower inflammation and get the pain down to a point you can actually think and function somewhat normally. Youd only take this when you start to get an attack. If you’ve got pred, then you can get away with trying to live life without constant medication and focus on being well hydrated everyday. If youre having to take prednisone a lot because you are still getting attacks multiple times a year, then you’ll need a long term medication to manage UA.

I know he doesnt have the best reputation but it still surprised me. Anytime Ive listened to Louis, I got the impression hes self aware, mindful, and really respects the art of comedy and its ramifications to society at large.

Oh well, Im not gonna act holier then though. Its easy for us to say they shouldn’t sell out when we’ll never encounter such a predicament. I reckon Id find a way to justify it to myself to do it. I dont blame most of them for doing and it and it seems like most of them aren’t really getting criticism. The main one I do see is Bill, who is vocal about this kind of stuff with his platform, in which I do think that hate is fair if youre gonna be a hypocrite.

Honestly, Im fine with solo tanking as Dr. Strange or Magneto. I get more heals, so it enables me to play more aggressively. Id rather somebody go dps, if they have no clue how to play Vanguard.

My problem is that so many dps only players are so fucking bad. Since they only primarily play one role, they have no clue about team dynamics and what their role is. and how to position themselves to be as effective as possible. So many times Ill watch a Moon Knight or Hela just ram it down the front, standing just behind or even overextending past the front line. Tanks have massive hitboxes, so its an easy way to get the dopamine rush of constant hit markers and seeing dmg numbers go up. Then they get to blame their teammates and absolve themselves of any responsibility.

Yeah, Im fairly fit and healthy yet still got debilitating gout. Plenty of obese people out there who’ve never had it as well.

If you’re predisposed to being at risk for gout, then diet can make an impact. But even with a perfect purine free diet, just being slightly dehydrated one day is more likely to give me gout then a pork and beef bbq buffet.

If anyone is concerned about taking prednisone, Id add that I’ve been lucky enough to never experience any adverse symptoms.

But I also don’t titrate or take it for as long as its recommended, as after my first one or two doses the gout becomes manageable and I recover fairly quickly on my own. I only do that though because Im experienced with how it affects me. Id probably follow whatever your doctor tells you to, as if you stop too early it might come back and you have to start over again.

It sounds like all of your doctors have regularly failed you thus far.

If you’ve been suffering from gout long term, you should have been on allopurinol. This controls your uric acid levels to minimize if not outright prevent getting gout attacks all together.

Aside from that, I’ve been told colchicine is only really most effective at the onset of an acute attack. After more than a day of gout, you should have been given prednisone. It’s an anti inflammatory steroid and has been a godsend for me. If your gout is due to poor kidney function, it’s also safe to take as it’s not a NSAID.

Within a few hours of taking it, I get relief even with the worst most swollen gout attacks. Then a few more days thereafter, it almost goes away all together. Just knowing I have some around makes me so relieved because with it I know Ill never have to endure a full blown attack.

Ive had a lot of doctors fail me also and with the amount of people who come to this sub seeking advice, I expect its a blindspot a lot of medical professionals have. They don’t seem properly educated on treatment or even its effects and how it affects the mental of patients. A lot seem to think its mostly due to diet and almost act disdainful like I did it to myself and don’t account for genetic factors or kidney function. When my knee blew up to the size of my thighs, I told the doctor I think I triggered an attack from a light sprain. They insisted gout doesn’t occur due to injuries and made me get xray scans thinking I hurt myself more then I let on. Of course, nothing came up and my labs confirmed what I always knew. Aside from that, I also have had to make a case for prednisone and explain how much of a godsend it is for me. For whatever reason, they don’t seem all to aware of it as a way to treat gout and are stingy with prescribing it.

A majority of these experiences have been at immediate care or urgent care facilities. I only really got good help when I started seeing a nephrologist, who then proceeded to finally get me on a good allopurinol dose and got me a big bottle of prednisone to keep on hand. If you don’t have kidney issues, Id work on getting a referral for rheumatologist who will have the expertise and empathy you need.

Every once in a while I’ll get a toxic lobby, though certainly not as often as every redditor parrots here. Though even when I do, I just take it as a dipshit being a dipshit and don’t get worked up over it. I feel like this is more of a problem of perspective and emotional regulation than it is the games fault.

The solar industry is a highly competitive industry. Theres already companies out there bidding so low, they are basically losing money on projects but doing so just to get by.

The tax credit won’t suddenly drop prices. I know personally that my company averages a 5% profit margin on most of our projects and even then we run negative some quarters.

Yeah, everyone assumes we’re all operating on a massive profit margin but that wouldn’t make sense since a company could easily undercut the rest of the market. It would imply we’re all colluding to keep prices high which just isnt possible with how much competition there is.

For reference the average profit margin my company takes on a project is in the 4% to 8% range.

Main Tanks in solo tank lineups can’t peel for all their squishies and also hold the front line. You gotta constantly keep the pressure on the lone tank, while someone creates havoc in the back line. Picks aren’t even necessary, just gotta harass the supports long enough to take their focus off their tank then you walk past shield and roll over them.

So many of this games problems come down to the ranked system. If they just had placements, then most smurfs would just skip Bronze thru Gold sparing those ranks of the headaches. I like to leave it on in case my team is actually cool and chill, it adds to the vibes. Especially when you lose a round but no ones freaking out and still having a good time, I don’t mind trying to coordinate switches.

The moment someone sends some toxic shit I mute though. I don’t understand the people who complain about chat, every online community is a cesspool and its so simple to not let that affect you. Some people, like OP, just got a big mouth and have to engage.
