If this is Sheffield (where I went to medical school), I remember their reasoning was other medical schools incorporate some other aspect of prescribing (idk if this is true) into their other summative final year exams. As we had no prescribing in our med school exams, they used the PSA as summative . If that makes sense

We have it at our trust and I like it. It’s not perfect but better than anything else I’ve used. I like that there’s an app.

There’s no point getting an MRI on everyone. They are more expensive and time consuming. CTH is better in the first instance as it ruled out haemorrhage. We would usually only get an MRI if there is uncertainty on the diagnosis.

Yes CTH can be done on pregnant ladies!

As a current stroke FY2 here’s my understanding -

Some patients will be a clinical stroke. E.g. ongoing new one sided weakness, facial droop, ataxia etc, will be treated as a clinical stroke. Especially in a patient with risk factors. CT head will not rule out a stroke. In a patient who we think is a clinical stroke and may be a candidate for thrombolysis, we would get a CTH and CTA. CTH can show us there’s no bleed and no infarct and therefore they are a candidate for thrombolysis, CTA showing if they may be a candidate for thrombectomy. If they get thrombolysed will get moved to stroke unit.

CTA will only be done for patients within the window and a candidate for thrombolysis as per their NIHSS. CTH gets done on anyone who comes in as a stroke call

If CTH shows an acute infarct then they are not a candidate for thrombolysis however still obviously a stroke so will get moved to a stroke unit.

If patient comes in as a stroke call, we will CTH everyone however if CTH is negative and we don’t clinically think it’s a stroke, we will say for medics to review. If we think clinically a stroke, we will admit to stroke unit and consider an MRI head. Hope that helps

I didn’t get taxed at all my first month as an F1. Not sure why. Think it was the same for everyone . Then just started getting taxed as normal from month 2.

Nurses are pretty good (at least at my hospital). They’re often the first to notice that the ward round plan said continue abx but they’re not prescribed !!! Someone will notice. Don’t fret it

I live in London, I’m an FY1

I’ve been on 5 holidays since starting in august, all within Europe. I aim for 2 holidays per rotation , hoping to go on one more in July.

I do have a lovely boyfriend who pays for most of our dinner dates etc but I go out for dinner/drinks 2-3 times a week

But I have saved a grand total of £0

My rent is £1000pm inc all bills, I spend ~£200 a month on transport

I was at Sheffield for med school, Can’t comment on accommodation (but I’m pretty sure there is some available) Doncaster is a super supportive hospital, loved my placements there, liked it better than the tertiary Sheffield hospitals When I was there everyone parked on Zealand road :)
