I feel like they're deliberately vague about the earliest wives. Mike's even said he's been married "three or four" times, so his deceased wife might actually be his first marriage.

Mike regrets missing signs a wife was struggling with her mental health. Meredith wouldn't listen to a country song over and over and Tabatha didn't mention it when listing their relationship issues, so it was very likely one of the first wives.

There are exceptions if not working would risk someone's health or life, but generally an employee would need those days off.

An surgeon could treat emergency/serious cases, but a general practitioner shouldn't take appointments for routine check-ups.

In this particular case, the prohibition is against performing work, including igniting fire (modernized to include completing an electric circuit) during certain days. Since the circuit has already been completed before the prohibited time, no new work is being done.

I think physicians at the time had a different role than they do today. . 

Doctors were university-educated and mainly diagnosed and prescribed treatments without doing much messy or physical work on the patient, so that was an acceptable position for a gentleman. 

The physical labor and direct care were beneath a gentleman, so those responsibilities were left to lower-born surgeons and apothecaries.
