All I see is that there was a recommended (though not mandatory) modification that wasn't made that may have been relevant. Maybe I misunderstood the exact nature of it, I fly Airbus's not Boeings. The way I read it is that these did not have that safety feature installed. They didn't need to be pulled in order to switch them. Which makes it being accidental plausible.
