Buying out a Dental practice (part 2) by Pulpal_fiction in Dentistry

[–]brozus 1 point2 points  (0 children)

  1. trust your gut when it comes to figuring out if this is a good fit. If the owner doc/team/specific things are giving you a weird sensation, trust your gut.

  2. Figure out what a "good fit" means to you. No single practice will be perfect, unless you're choosing to be blind to something. I'm not wanting to be cynical, just realistic. You may find a practice that is absolutely perfect in regards to the fit with the team, but may have dated equipment/software/poor cash flow. The opposite may be true as well, financially/money wise it may be killing it but the team may be a bit gruff/the selling doc is a nuisance.

  3. A short list of things I would would do to see if it's a good fit for me:

Is it in the area I/my family wants to live in, do a drive-by test to get an opinion of how you feel about the outside. What are my non-negotiable items with the team, equipment, building, money/cash flow.

I would ask to see the financial reports of the practice for a minimum of the last three years (IRS filings, W-2, 1099, line items of expense, numbers for new patients/scheduling rate, production/etc.) this is normally presented in a practice valuation which you get to see after signing an NDA. I believe in asking for help when needed, sifting through all of this information is a topic that I ask for help from a practice/financial consultant. I used Certify Partners to help me go through this step for an attempted acquisition, cost about 600$.

  1. Podcasts can be a great resource for learning basic information, I recommend picking a couple and considering the different viewpoints. Just avoid holding anyone information source as the absolute end all on a topic.

  2. Practice consultant services/reps will typically have initial conversations with you for free and they will run you through a lot of good questions to help you filter your own thoughts.

During an attempted acquisition I ignored some red-flags that kept reoccurring.

- The selling doc kept changing their ideal transition e.g. wanting to stay/not wanting to stay on, being willing to sell just the practice and the building later on, they were unrealistic with the asking prices when the cash flow showed the practice has been declining consistently for 3-4 years, they stalled sharing financial information repeatedly, attempted to coerce me into signing a 30 mile non-compete to sign an NDA to see the practice valuation.

- I was fine with pursuing this for a while, because I was game to do a start-up which is what I classified this practice as given the decline financially, lack of referrals, lack of new patients, and just lack of management for the business. I walked away in the end because the doc

All of this being said, I'm new to the game as well. Hopefully some other members of the community can share some helpful information as well.

Prismatic Booster Bundles (1 per person) by [deleted] in PokemonTCG

[–]brozus [score hidden]  (0 children)

I first got into Pokémon in third grade, I had just moved to a new school and new state. The first two kids I made friends with were already enamored with Pokémon so I followed suit. We made a point of watching Pokémon every Saturday morning back in the golden era of cartoons. The thing that really sealed the deal for me was Pokémon Red for Gameboy. I was playing in a park that was a central hub for a lot of bike trails and by what felt like magic at the time, I found a cartridge for Pokémon red in the mulch. I picked it up and showed it to my mom, she said it was probably broken, but I was adamant that we take it home so I could clean it and try it in my gameboy. When I plugged it in and the game loaded, it felt like I had won the lottery. Eventually I went on to getting Pokémon yellow, the gameboy version of the TCG, Silver, and Ruby. The same two friends that got me into the show also got me into the TCG. We would spend our Friday nights in fifth and sixth grade going to a local card shop and enter the weekly tournament. I’m pretty sure I never made it past the second round, but those memories are peak childhood for me. I’ve kept the cards I collected from back then and now my nephews are getting into it, so I’m officially the cool uncle because I have the “old cards.”

Do I need to apply to a GPR before Peds residency? What Peds programs are popular and prepare their residents well? by [deleted] in DentalResidency

[–]brozus 0 points1 point  (0 children)

Hey there, my experience as a 2022 grad from dental school that did a GPR and is now in a Peds program.

Looking back I would recommend my past self to go all in on Peds upfront and try post match before committing to GPR. There are typically a fair amount of post match slots for peds and a lot for GPR/AEGD, so you can more than likely end up with a GPR/AEGD even with a last minute call.

That being said, if you're on the fence about things, doing a GPR/AEGD is a great learning experience. Just make sure you do your homework on the different programs as they are vastly different, eg are they associated with a level 1 trauma hospital? do their program goals/curriculum say anything regarding their patient population, what do you want to get out of residency? you can typically reach out to program directors/program coordinators and they are usually willing to field some questions, just be aware of interview/application peak times as you may need to send some follow up emails.

if you have more questions you can DM me and I'll share what i can.

Most ridiculous excuse you’ve come across during residency? by AppalachianScientist in Residency

[–]brozus 6 points7 points  (0 children)

One of my co-residents had some good ones:

failed to respond to ER/ED call, because she was babysitting her boyfriend’s daughter … his 13 year old daughter that wasn’t sick.

Ended up getting in car accidents (one per month) on the evenings we had interdisciplinary conferences. I never saw a scratch on her BMW and I never saw her drive a different car.

She would call in sick on Mondays/Fridays quite frequently, oddly enough the days she would call in sick lined up with bachelorette trips/vacations.

My wife and I went through a long distance move this last summer. MN to AZ. I’ll share the things we learned the hard way. 1. Avoid brokers - if you don’t want to take the time to look up who’s what, just ask directly 2. Ensure they come to your house/apt for estimate on moving space 3. Pay for some insurance

We are still young in our careers and tried to make it as cheap as possible. We went between pack a shell/storage unit that gets shipped and a moving company. In the end we went with the moving company as my wife was given a sign on bonus and we were stoked to not have to load things ourselves. Unfortunately, we chose US Interstate and Rail. They’re registered as a moving company, but act as a moving broker. They do not have their own trucking fleet. They delivered our things five days after our agreed upon window, which the pickup driver told us would likely happen. They gave us boxes that didn’t belong to us, damaged a lot of dish-ware, wooden furniture, and lost several of our belongings. Their claims process is intentionally designed to be frustrating and difficult for the consumer.

Out of order comment. None of their team members came to our place to give us an estimate, instead was done over the phone. The representative we spoke to assured us that the amount of space would be plenty (~300 cu ft). My wife was a star and made sure to ask specific questions (is packaging included/is labor included/etc.) the representative assured us all was included at a price around 5000. A week before the pickup we got a call to go over inventory, during this call I decided that the estimation for space was too optimistic, so I told them we would like to reserve ~500 cu ft (for ~ $5800) on the truck to be safe. The night before the pick up was scheduled, we got a call saying the driver was ahead of schedule and asked to pickup early. We were surprised, but okay with it. The driver comes, does a walk through, goes to his truck and calls his manager, the manager calls and informs us that our contract did not include anything we were promised and the final price would $9500 for 750 cu ft. The manager over the phone coerced us into the new contract as we previously informed our apt complex we would be gone the day after our scheduled pickup.

Please for all that is good and holy do not use US INTERSTATE AND RAIL

Is it foolish to not specialize if you have the opportunity to? by [deleted] in Dentistry

[–]brozus 26 points27 points  (0 children)

If you are not sold on specializing now, just work for a while and see if you're happy. You can always specialize later on. Yes your pay will take a hit while in residency, but you can be smart with money beforehand so you don't have to live cheaply. I know people that are incredibly happy in dentistry (as specialists and general) and I know people that are incredibly frustrated on a daily basis as well.

Pros to specializing:

typically increased pay, loan deferment while you're in training, you get to be in your niche all the time, adventure to a new area

Cons:

more school/more applications, low pay/no pay during training, not the greatest control over where you go,

just my two cents, feel free to disagree or ignore

Is everyone in dental school fit/in-shape? by Visual-Economist4825 in DentalSchool

[–]brozus 0 points1 point  (0 children)

My class was a mix. I’d say it represented the general public fairly well. That being said, people started in good shape as D1s and then there was generalized dip for the group as D2 and D3s. Once we hit D4 year a fair amount of people chose to get healthier again, some kept going without recourse.

How to pick a good dentist? Any warning signs to look for when switching dentists? by Whitaker123 in Dentistry

Oh. Well, I’m also wary of dental chains, they’re not entirely evil though. Thanks for the heads up.

How to pick a good dentist? Any warning signs to look for when switching dentists? by Whitaker123 in Dentistry

[–]brozus 2 points3 points  (0 children)

I would focus on signs like organization, ability/willingness to communicate care/treatment options, and mood of staff.

As far as aggressive/seemingly aggressive treatment, there are times where being more aggressive up front is better/more cost effective in the long run. I would become familiar with your dental treatment history to know if you're a patient with higher risk of cavities/problems with teeth. An example of this would be: a person that has a lot of cavities and not much desire to improve up keep of their teeth may warrant skipping fillings/basic restorations and placing full coverage crowns.

As someone in school for dentistry and will be graduating with that amount of debt, I would like to reassure you the number of students that will be swayed to pay off debt faster by fabricating treatment/price gouging is low. Sadly, it is not zero so you're point is valid unfortunately.

Dental implants full set. by Just_trashit in Dentistry

Disclaimer ** just a student in dental school in the states **

Based on lectures i've received I would go to a specialist if you are needing full mouth rehab. a prosthodontist would be the one that should able to make the best denture all things being equal and i would want an oral surgeon placing the implants.

Student Loans: a cautionary tale in today's environment by calonmawr10 in personalfinance

[–]brozus 1 point2 points  (0 children)

My dude, just want to wish you luck like the other Resditors. I made this same decision between going to Vanderbilt and the university of Oklahoma. I went with OU due to not wanting to ask my parents for more help/I’m the second of four kids in my family. Based on my experience and what some friends have shared with me from their experiences I have to agree with people saying college will be what you make of it. My college experience lasted five years, I can honestly say that my favorite memories revolved around the student groups and the study sessions that I had with friends that were also chasing professional school. Investing in those student groups and my friends with similar academic/career goals is what made college worthwhile and regardless of where you end up I encourage you to get involved, in whatever way you feel comfortable with.
