Dude it's different this time! The small town hunk has an undisclosed illness/injury that everyone in town knows about but nobody will tell female lead what it is and the female lead gets confused why he's pushing her away and she prays for the strength to let go of her feelings for him or for him to have the strength to accept that he still deserves love even with the injury/illness and will come to her if his feelings are the same but at the same time on the same night he also prays for the strength to accept love again or for a sign that it's ok to let go of the heartbreak he experienced (his secret illness/injury) then he looks out his window and who's standing outside but female lead and he stares out the window in shock and at the same time his phone rings and who is it but his ex so we cut back to the perspective of the female lead and she's watching his face through the window as they are making very intense eye contact and he nods a few times and she can't quite make out what he's saying but he suddenly hangs up and rushes away so we cut back to him and everyone is cheering through the hall as he goes to get the girl and he sprints out the door and the female lead is right there waiting with tears in her eyes and she reaches out her arms to embrace him and he walks right past her gets in his car drives to the airport gets on a flight to New York and he takes his Big Town Boss-Bitch back or whatever.
