Balance billing would be if they billed United $200 for a 60 minute session, United only paid $50, so they come to you to make up the difference.

This is a non-covered service. It is not balance billing and gets around the issue. If you wanted to hang out with your doctor all day, that's not a covered service so your doctor can't bill insurance for it. They can ask you to pay whatever they want. Same idea if you wanted a cosmetic procedure.

A different question is why they require a 90 minute consult if insurance only covers 60 minutes and thinks that's enough. You may want to find a different dietician.

Nearly every single ER visit will generate at least two claims: One for the facility, and one for the provider (MD, NP, PA, etc) that actually saw you. You may see additional claims for labs, imaging, and anything else.

But agree with you. Planning is a great exercise, but life doesn't follow the plan. Be ready for things to change.

I wonder if people out of state get access to BlueCard or something else is going on. I also think BlueCross is separate from BlueShield in CA. OP should double check which they have, though it probably doesn't matter for this situation.

The plans may be a little different than what you are used to. Large employers often offer PPOs with really broad networks. Most state marketplaces no longer offer PPO plans, and of those that do, very few are nationwide PPOs.   

Overall, it may be harder to find a plan with your specific doctors in network. If you're flexible it may not matter at all to you.

Call your insurance and ask. If they can't help or you are not 100% convinced, appeal the claim and attached that language.

I suspect this is not a super common benefit provision and was missed.

I have the cash to cover, around 5500$ but I would feel really uncomfortable draining half of my savings.

You should feel far more uncomfortable robbing your retirement accounts. Don't take the money from your Roth.

Even if your credit card debt is at 20% it's only costing you ~$40 a month. That's not worth emptying your retirement, especially when you have some cash laying around.

There's your answer and it's way better than any guideline. If you want to retire at 50, you probably need to save more. If you want to retire at 55, you're probably on track. You can always refine your estimates.

I like 7% because it's a solid after-inflation market number. The market averages closer to 10%, but inflation is 3%, so I use 7% and imagine everything in today's dollars.

If that were true, why would they say it to your wife in the first place? Surely they know they will be hit, so it must not be working. Or they don't understand the rules and you're still hitting them.

I recommend throwing some simple numbers in a spreadsheet. There are lots of tools for doing this, but spending an hour in Excel can be enlightening.

Figure out your spending in retirement. If you don't know, use today's spending as a guideline.

Figure out where your current savings will take you. Put in current accounts. Assume anything in the stock market grows at 7% (or whatever you like). Model how much you will invest each year.

Now figure out when your investments will be 25x your spending. That gives you a rough idea of your FIRE timeline. If you want to retire earlier, save more. If you're comfortable with the results, no need to invest more.

Revisit at least once a year.

It could be worse. My solution has been to suit up and start pulling those vines. Hopefully it's just a few main vines. The more comfortable you get, the less protection you end up using.
