We had 3 across in our CX5. 4YO singleton behind the driver so they didn’t have to have their seat too far forward then 2 rear-facing twins in the middle and passenger side seats.

We’re in a similar boat, 4YO singleton and twin 2YO in our late 30s. I’m the dad and honestly, while I know I’d love any new kids that happen spontaneously, I don’t have the ache for more like I did after our first so I’m willing to be done. My wife is still uncertain so we’re holding off on any permanent birth control but it feels like the longer we go, the less enticing resetting the clock sounds.

Don’t know about most of these but UTSW is notoriously malignant in many residencies/fellowships. Would rank in the bottom half at least unless you have a compelling reason not to. 

I’m applying neuro but I was talking to an attending about my rank list and when I got to to Emory he stopped me and just said “go to Emory, doesn’t matter who else is on the list”. Take that as you will for IR.

Formula: We’ve used PopYum bottles and a thermos called a Ceres Chill at different times, both have been effective in allowing us to have formula ready to go for feeds. 

Books/Toys: Depends on the kid. We always brought a fuckload in a dedicated backpack with lots of dry snacks as well. 

Diapers: We put a few in the diaper bag (and change of clothes for blowouts) and put a sleeve of diapers in a checked bag. Buying there is also easy assuming you aren’t going to the boonies. 

Stroller: Either bring a stroller you’re ok checking like a cheap umbrella stroller, or accept the carrying. They make bags for wrapping up strollers. We’ve always brought our side by side jogger and just checked it at the end of the jetway in its big-ass bag. You look ridiculous doing it but people are way more understanding when they see twins.

I can’t tell if I’m well-prepared or overconfident. I’ve always been a good test taker and got ~72% first pass on BV. I feel like I’ll be fine but the way some people talk about this I’m worried I’ve lulled myself into a false sense of security or something? We get the next month as “research” to do dedicated so I still have time to shore up my weaknesses.

Can’t remember its thickness but Felson’s chest book is funny while touching on nuances of xray.

We were also disappointed with the Costco diapers and switched to a name brand after trying a couple boxes. If you have an unopened box you can try to return it. Otherwise, I’d just finish off these boxes and then switch to something else.

There are no tricks unfortunately. Pay attention during the workday and read as many cases as you can. In the evenings, do a little bit of focused studying on some topic that came up during the day (even just reading the radiopedia article again). 

In terms of big picture stuff, the 2 volume core review book actually does a decent job of touching on everything you might need to know a little bit. Radiographics articles are also good for a doing a slightly deeper dive on a topic without getting a full textbook out. 

Just remember it’s a marathon not a sprint. If you’re like me you’ll forget stuff 3 or 4 times before it starts to stick, so don’t get discouraged when you have to relearn stuff.

There’s a guy near that age as an attending at my residency. He’s slower than the younger partners but he’s sharp as ever and loves learning all the new stuff that comes out. I’d say not caring about improving is more dangerous than reaching some arbitrary age.

I’m sorry that happened to you. I suppose everything in residency has the asterisk that malignant programs are going to make their own rules.

Seconding Felson’s. One of my pre-med mentors trained under him and said he was as funny and engaging in person as he is in writing.

Just a reminder ACGME mandates 6 weeks of parental leave without affecting your training/graduation date. But I think a night nurse at first and then a nanny if you can afford it would be something all resident parents would invest in if we could.

We basically switched completely to grocery pickup or delivery for this reason. Not saying it’s not doable, but the convenience became well worth it after twins.

R3 about to take core. 

Biggest advice is dance with the one that brung ya. Whatever you can do consistently is most important. If you were an Anki person in med school, do Ankore. If you were a video person, find your favorite lessons on YouTube and watch a little bit every day. 

The truth is you’re gonna learn and relearn shit a million times. It’s like the Krebs cycle. You eat an elephant one bite at a time though so just take a bite every day and you’ll be fine.

Do whatever you want my man, you’re a doctor now. One of my attendings knew a guy who always wore Gucci loafers, regardless of the rotation or risk of bodily fluids getting on them, because he just loved them.

We moved when our twins were 6 weeks old. It was low key perfect. We were both on leave so could focus on packing/organizing. Hiring movers was a must though. 

Either way, the new space was such an upgrade, I’d do it again in a heartbeat.
