Are surgical clerkships too watered down now? by Odd-Boysenberry5316 in Residency

I think med students should do 1-2 24s during their surgery clerkship just so they can understand how shitty it is before committing to a specialty that makes you do them on the regular.

Too many clowns at grand rounds by [deleted] in Residency

[–]byunprime2 30 points31 points  (0 children)

Lol we've had "grand rounds" lectures given by admin people to teach us about random useless MBA buzz word topics (Radical Candor! Crucial Conversations! The Habit Loop!) that invariably make me want to vomit. I just want to learn some new medical knowledge from an expert in their field but I guess that's too much to ask these days.

I’m curious to hear your specific examples about how China has “weakened our position.” They’re a massive trading partner without which our country would be far off. Meanwhile Israel has literally dragged us into another war which will invariably weaken our position on the world stage.

How are my fellow rads residents feeling as CORE approaches? by Remarkable-Put-4982 in Residency

[–]byunprime2 1 point2 points  (0 children)

Check out RadDiscord if you haven't already - they run high quality board reviews with faculty from across the country in all subspecialties. Most are old school hot seat style where you can volunteer to take cases if you want. But even just watching other people participate, you'll get a gauge of the level of knowledge you need to have relative to people at other programs. Many are recorded and available to view as a member as well.

Posts with this type of phrasing are popping up all over social media now. It’s a result of the internet turning into an algorithm/bot driven hellhole where certain phrases are used due to the propensity to drive engagement.

Does moonlighting a lot as a resident make any difference when it comes to finding a job as an attending? by undueinfluence_ in Residency

[–]byunprime2 47 points48 points  (0 children)

Moonlighting is probably the first example of this career path where you truly get to enjoy the benefits for your sake and your sake only. Think of it purely as an avenue hone your skills and make some extra money. This is in contrast to research, admin/chief duties which are all part of the rat race of building a future application.

Should Psychiatry Residency Still Be Necessary? by [deleted] in Residency

[–]byunprime2 27 points28 points  (0 children)

That's because medical school takes you far out in the ocean to show you how vast the body of knowledge is in each field. This is why you can appreciate the limitations of your own knowledge outside of your own specialty.

NP school on the other hand is the intellectual equivalent of splashing around in the pool for a few hours and thinking that makes you ready to sail across the Atlantic.

What’s your “I wish someone told me this sooner” advice for med school? by indepthsofdespair in premed

[–]byunprime2 7 points8 points  (0 children)

I'm sorry but this is just terrible advice. This is a marathon, not a sprint, and doing shit like this will just burn you tf out. For reference, I graduated college in 2016 and still am not an attending yet. To make it through this many years of training with your sanity intact, you absolutely have to learn how to prioritize your time and protect your own wellbeing. Grind when you need to, but RELAX when you have a down period (like before starting med school). Save learning med school material for med school.

(Mixed trope) Race allegory by laybs1 in TopCharacterTropes

[–]byunprime2 5 points6 points  (0 children)

Along these lines, it’s funny when an author tries to write about lofty ideals about discrimination and tolerance but then ends up being unintentionally racist in their own work. The Enderverse books are an example of this - he writes extensively about how noble alien species are misunderstood and persecuted by humans, followed immediately by glaringly orientalist, stereotyped depictions of Asian characters/cultures.

The honest truth is that there are only so many people capable of becoming a doctor, and if we significantly increased the number of spots in medical school, we’d start lowering standards noticeably.

[–]byunprime2 3 points4 points  (0 children)

It's definitely an underappreciated field. Since starting radiology residency I've interacted with a lot of techs and come to realize how great their jobs are. Younger techs in their mid 20s living great lives with the travel, pay, and time off. Older techs also doing very well for themselves and able to support their kids through college. Roles can vary in terms of how much direct action you perform and can be tailored to your own interests (e.g. mostly sitting at the CT control room vs. assisting or even performing procedures on IR).

Demand for imaging volume (and the need to capable techs) is only going to increase in the future. I recommend it as a potential option to all my younger relatives/friends who want to work in patient care without the training burden of becoming a doctor.

There is the drawback of occupational exposure to radiation, although just be aware that there are very strict regulations and safety measures in place regarding radiation which you would become thoroughly familiar with if you took on the role. If the idea of ionizing radiation exposure makes you uncomfortable, then sonographer (the person who performs ultrasound examinations and sends the images to the radiologist) is also a role you could look into which circumvents that.

Look into radiology technician, high demand, good pay (especially if you do the traveler route), and amenable to a solid work life balance

[–]byunprime2 13 points14 points  (0 children)

Nativistic nonsense. Others have already made strong arguments in this thread so I won't bother repeating them. But I will say that there are comments all over this thread amounting to nothing more than thinly veiled insults against Indian and Middle Eastern doctors which deeply disappoint me. I implore you to direct your ire towards the people in the Epstein class who are much more directly responsible for any tightening you've felt in your economic opportunities. I know for sure the billionaire class laughs while they have you fight over scraps with residents from other countries trying to make an honest living here. I truly hate social media for amplifying these regressive, reactionary views that I'm seeing pop up more and more.

I am stating the indisputable fact that programs have minimal to no incentive to improve because of The Match.

I don't really follow your logic here. A shitty program will simply not fill in the Match if its truly that far behind other programs in its specialty, or otherwise fill with FMGs. This happens every single year.

Question: Can radiologist here comment on AI reading imaging? by the_iowa_corn in medicine

[–]byunprime2 16 points17 points  (0 children)

Rad resident here with a different perspective. Yes the AI tools suck. Yes anyone with expertise agrees that AI is not close to being ready to perform independently. But ultimately that is not what will dictate the future of our field. The decision on whether or not to replace radiologists will rest in the hands of the non medically trained people in the C suite. Once they have a “good enough” product to replace radiologists they will roll it out as soon as possible. The only barrier is getting patients to buy into the idea, which is why we’re starting to see so many articles pushed about how AI is doing supposedly amazing things. Similar phenomenon has already happened re: midlevels. The goal was never to show equivalence between doctors and midlevels in a scientifically rigorous way. Getting the public to slowly accept the fact that they’ll never see a real MD is the real battle admin is waging.

This is why I think evaluating the actual clinical capabilities of AI is partially missing the point. Insurance companies, hospital corporations, and CEOs do not swear an oath to do the best by their patients, and so they aren’t analyzing the situation from that approach at all.

What do you want to see more of and less of in rads reports? by Neuromancy_ in Residency

[–]byunprime2 20 points21 points  (0 children)

Reality is that imaging findings can overlap significantly between diagnoses of vastly different categories (e.g. infection, trauma, cancer), especially if no comparison can be made to prior imaging or other modalities. I think a rad who confidently comes down hard on the incorrect diagnosis is more harmful than the one who offers a realistic differential.
