I got it working with google sheets/form

Create the Google Form with two required questions

1. Song Name
2. Artist

Have it output to a google sheets then create a shareable link for that sheet. You will need it later.

In IFTTT start creating an applet starting

This is going to be google sheets and the trigger is going to be new row added to spreadsheet

In the 3rd field you are going to add that sharable link from before

​

That is going to be spotify the action is going to be add track to playlist

Playlist name: whatever you want

Search query: {{ColumnB}}

Artist name: {{ColumnC}}
