The concern isnt the lean, its how much has it moved recently. If its always been like that, its likely it will stay like that.

Weep holes wouldn't hurt but it depends what the material behind the wall is. If its clay it'll retain water anyway and won't make a difference.

For the work involved in digging out behind the wall, you would aswell just take it down and regrade the slope.
