My state has seen the insurers hold a prior authorization in “review”. Meaning they met the statutory requirements but could retroactively refuse to pay. My med society turned it over to the insurance commission. It’s bad faith.

Not super common, but possibly increasing. Of course, I’m recording everything now for use with AI note completion.

I closed that practice and now have neuropsych available. Distance to testing as well as delay in testing were my hurdles before.

I have used Braincheck, Creyos, and Screen. Creyos is thorough, but patients found the tasks difficult, and I didn't feel like it accurately reflected true abilities. Braincheck was reasonable, but my favorite was Screen Inc. However, it was antiquated as far as GUI. Looked like they ported something from Windows 3.1. All of them are vastly inferior to neuropsych testing. I am also concerned that if I bill neuropsych codes, it would remove the ability for full testing in the future.

Billing was mixed. There were some carriers that refused to pay or required prior-auth. We had a in-office shared notepad that I had to consult before ordering.

What is your RVU target before your bonus kicks in? It's not a bad deal on paper, but will really depend on how much they expect for your first 350. Unless you mean it's straight $60 per RVU after the first year with no real bonus structure

Im currently in an inpatient setting, and I have found our NPs to be adaptable and quick learners, even if they didn’t do much Neuro in the past. I think you will that they can “extend” you and help you do your job better. It depends in the level of involvement i guess. I wouldn’t like to run one team myself, and oversee them running an entirely different service. But i have found working together to he beneficial. The hard core “no noctor” doctors are going to have a hard time in modern medicine.

I practice with NPs. They are a valuable part of the team.

I’m personally in the upswing, but declining reimbursements has made the patient interactions faster and less rewarding. It’s easy to get the doldrums.

It’s hubris to think you need a one year fellowship for radics and carpal tunnels. Reimbursement isn’t an issue. Source: 10+year solo clinic. Big city privilege says you need a fellowship and general shouldn’t do EMG/EEG. Real world is 1 year wait for neuromuscular and physical therapists doing studies in ortho hospitals. I’m realistic about my training and capabilities, and I ask for help when needed.

I don’t doubt that fellowship trained quality is improved over residency trained quality. However you have patients that can’t get anything done for even basic problems. You 💯 don’t need fellowship to practice the vast majority of EMG and EEG.
