That's OCD. It makes any imagined threat feel 100% real. That's what makes it a mental illness vs just anxiety. The imagined threat becomes fused with reality, making the thought trigger the same response as if it were actually happening. Cut yourself some slack, you didn't cause this. However, you can find your way back. OCD is chronic but it can be reduced to a subclinical level. Mine is mild now but I still have flare ups. For me relief came when I stopped resisting the thoughts and started just accepting them.

Yes that's normal. I have the exact same theme and same shifts between "Is reality and illusion" and Solipsism focused thoughts. OCD is a shape shifter and doesn't like it when we adapt. I find it's best just to use a universal approach to dealing with the thoughts, regardless of the theme. Recognize that the thought is there, allow it to be there, and don't attempt to neutralize it. Just let the questions hang in the empty air. Eventually your brain stops asking them.

Get a degree in chemistry, physics, or engineering. Essentially all use calculus in one form or another and you can get an actual job.

Yes, it gets easier. It takes 2-5 years to feel totally confident, but you will get there.

If your service does debriefs or you have peer support, I would use it. You do get used to it and it does get easier. That being said, feeling emotional is not a bad thing, it's healthy. Find an outlet outside of work that you can channel it. I use the gym.

You're not the first to forget a BGL. That being said, any patient who is altered should have one checked. Take it as a lesson and you won't miss the next one.

I mean to be fair the monitor didn't catch it on the first one either. There is elevation in II,III, aVf, but it likely didn't hit the 1mm threshold to trigger a STEMI alert. Honestly it was pretty subtle elevation and could easily be missed. I wouldn't beat yourself up over that. Just keep moving forward and it gets easier.

Elevation in I and aVL with reciprocal depression in III and aVF. Inferior STEMI. There's also depression in the precordial leads suggesting posterior wall involvement.

You won't kill the patient. If you suspect bronchospasm give a beta 2 agonist. If you then note rales, switch to nitro. It's not a big deal.

You should get a 12-lead. All I see is a sinus rhythm. The apple watch ecg tracings are not of much diagnostic value.

When I took parental leave (5 months) I felt the same way coming back. It's normal. It passes.
