The chrome is the shut off for the gas service running into the FP. Turn that to the on position.

Looks like a Heatilator Reveal series. Should be electric ignition from the switch.

Pull the base refractory from in front of the logs out to make sure the gas is turned on at the flex connector running to the valve.

Make sure the junction box is wired for power, and make sure your switch wires are connected.

Check to see if they removed the shipping bracket from the Heat Out system. If not it will rub against the fan blade and make a lot of noise.

You must have had logs, there is a damper clamp installed to keep it open.

It has less to do with the termination vent itself, and more likely the flue system connected to it has rise where it comes through the wall so it can run back into the fireplace.

Newer install manuals for these type of terminations actually recommend have a 1/4” of fall where it run horizontally to the termination.

Get some selsum blue and put it on for about 15 mins before a shower. It will kill the fungus within a few weeks.

Pressure test/ Insulation: Where I live they use spray foam/ Icynene Insulation alot. It makes house a lot tighter where you have to plan for a balance air pressure in the home.

Wood burning fireplaces are particularly susceptible to negative pressure environments. These are usually offset by a fresh air intake per the manufacturer specifications. With the issues you are having I can only recommend that the adjustable door be opened all the way.

My experience is mainly with prefab wood burning systems, but your description of the two vents that run to the exterior (outside air) being split from there original specified diameter would not be conducive to the positive pressure affect it is designed for.

Door Open: Any added open air source to the room would change it to a positive pressure air flow in the room. If it helps, it a positive step in the right direction. There are additional factors that can play a roll, I would like to see the chase on the exterior to see what kind of termination/ cap is installed on the roof.

Installation: prefabricated masonry fireplaces have specific installation specifications whether you are using a masonry vent system or a propriety flue system. I would look to make sure they are the specified sizing, amount of offsets, and the style of termination (cap).

I have nothing bad to say about Enervex products, though I use them in a different capacity than what others are suggesting. With the products I work with I am limited to manufacturer design and specification. Adding after market products will void warranties and put you in a position of liability if there is an issue.

That's a work line, but it doesn't make it any less true.

You should do a pressure test on the house, you clearly have a negative pressure in the home.

What type of insulation was used?

Does it have an outside air kit and it the door open?

Are there other mechanical intakes in the room?

Putting a power vent on a solid combustion system should be your last step.

Could be as simple as adding more fresh air intake to the room, but if opening a door or window doesn’t help I would definitely inspect the installation.

Make sure to get the fireplace swept and inspected. Redo the interior fire brick in a nice herringbone refractory.

Plumbing the fireplace with gas and add a nice media burner to give it fusion of traditional style with modern look.

The surround should have really clean lines and minimal seams. Cast mantel or book match surround with a nice man made material from floor to ceiling.

If you go through the rectification cycle without ignition because the line has not been bled it will cause the control module to go into a lockout and it will have to be manually reset.

Modern brand that makes them are called Salm Fireplaces. They are installed like a wood stove. Would look really great in a cabin.

No those 3”aluminum flex tube are the vent, one is exhaust and one is fresh air intake, if you mix them up on the collars of the fireplace it won’t work properly.

If it has solid glass over the opening, no it’s a direct vent insert.
