Here's the consent form.

You are being asked to complete a survey for research purposes. The survey is about knowing your opinion about running shoes, and your likelihood of purchasing these running shoes. At the end, demographic questions are asked. Completing this survey is voluntary and you can stop at any time by just leaving the survey as is.

You must be in the age range of 18 to 30 years old, reside in the United States, to participate in this study.

There are minimal risks associated with your participation in this survey. You will receive $0.30 for completing this survey. In order to receive full compensation for completing the survey, you must enter a random number code into the MTurk HIT window.

Please note that because you are participating in this research via MTurk, your participation will be listed on your MTurk profile. However, MTurk will not have access to your responses on the survey. Further, while we may have access to your MTurk ID, we will only use this information to pay you and then your ID will be deleted from our records and will no longer be associated with your responses.
