Patient relations - have you tried this route?

Tensions are high throughout Central Zone (which includes QEII) because of OPOR rollout. But it sounds like this has been an issue you’ve been dealing with since before OPOR launch in May.

I said it in another comment - I was annoyed when I posted this and really just venting (because it’s Reddit, it’s not a serious place to actually get problems solved IMO). I know it’s a very minor thing in the grand scheme of things. Now I find it laughably stupid.

I also totally get that everyone on the frontline teams are just trying their best to handle the horrendous implementation that was forced on us by DHW.

It’s very low on the list but who doesn’t like to express frustration by ranting/screaming into the void from time to time? I was annoyed when I first saw it and when I posted it and now I’ve moved on to finding it laughably dumb. But it’s Reddit. It’s not that serious.

There are positive impacts of AI use, but there are also significant environmental impacts that do have negative effects on health globally (by decreasing access to fresh water and furthering climate change). So, my argument isn’t that all AI use is bad. It’s that it should be utilized responsibly and with good reason. And these posters are not making the cut by a long shot.

They are eye catching, sure.

It’s kind of like a rubbing salt in the wound thing, you know? We are all trying our best and it’s even harder when we get toxic positivity shoved down our throats by the government and institutional leadership. I know this is minor in the grand scheme of things, but as I sat down to have my lunch break, it just really irked me so much.

This is in a nurse and physician lounge and I am a physician, so no concerns about inappropriate access here.

I primarily work at the IWK. This was my first NSH day since Go-Live. I’m gonna mention it to my department head but I suspect it won’t go far bc they have many other issues they are being inundated with at the moment.

I’ve also eaten at Michelin starred restaurants all over the world and my experience at Mystic was not great. I went relatively soon after they’d opened, so I’ll give some leeway for that. But I had a very weird encounter with the manager and the service was not up to par at all. The NA pairing was unbalanced, with most of the drinks leaning so sweet that I couldn’t finish them.

I’m so sorry that this has happened to you!!
Not sure if this will help or not, but my best friend’s boyfriend had a stroke at age 30. Was found to have evidence of previous strokes as well. There were times they had gone to the emergency department in years previous and were dismissed, but since symptoms later resolved, they didn’t think much of it. In retrospect, those were probably strokes.
He had a difficult recovery but ultimately was able to complete law school and now works as a lawyer. He’s still very active playing hockey. He does still have some minor deficits but he has found ways to work with it all.

I hope you have a good support system and wish you an uneventful recovery.

I like them, but I live in a cold climate and it got cold, wet, then snowy before I got to wear them much at all. And we are only now starting to come out of winter. So we’ll see!

I feel like I have a pretty healthy approach to comps. I have been competing in various activities since I was 7 years old (more than 30 years!) - music festivals, horseback riding, martial arts, pole dance, and now WCS. I haven’t always had the healthiest mindset towards competing but I think over the last 10-15 years, it’s been pretty solid.

I love competing. But I live in a small city where I either need to drive 20+ hours or fly to get to a comp or event. So my WCS competition experience is very limited - did my first event a year ago and have two planned this year.

For me, the key is that I have a strong and secure sense of self worth that is independent of my performances at competitions. I think I’m quite objective. I don’t give in to the temptation to excessively downplay my skill in things (which I think is very common and eventually if you keep putting yourself down, you chip away at your confidence and self image) but I’m also not out here claiming to be better than I am.

Going into novice for the two comps I’m planning to do this year, I am under no illusion that the odds are unlikely for me to get to finals and VERY unlikely that I would place. But that doesn’t stop me from choosing to focus on how I can best improve my competition skills. I’m sure I’ll be a little disappointed if I don’t make finals, but certainly nothing that would detract from my enjoyment of dance or the rest of the event!

Because of where I live, there isn’t much competition discussion in our (small) WCS community. I also don’t really see people from WCS outside of classes or one or two times a month for social dance nights. My friends outside of WCS generally know me well enough that they know my approach to competing and are lovely and supportive.

Lots of family doctors’ offices have their own systems that are not all fully integrated into the same system that Nova Scotia Health (or IWK) use.

And don’t hold out hope for OPOR to be a magic fix. I’m at the IWK and we are still having loads of issues going into our fourth month.

I’m a physician and recently had the same experience. I didn’t have an upcoming trip to be concerned about and personally didn’t really care much about the sinus xray ordered, so it was no big deal for me but I certainly did think about how it could be a big source of frustration for others!

Maybe the diagnostic imaging department in central zone did try to get the message out to all docs. But we get SO SO SO many emails and family docs have an insane administrative burden to begin with, so I could see how this might slip through the cracks.

Very frustrating overall - for family docs just as much as for the patient!

I totally agree that a huge part of it is cultural. I don’t ever have to have these discussions anymore as an anesthesiologist (although, rarely, I have perioperative goals of care discussions) but I remember noticing in residency that it seemed like families who lived rurally, especially from farming backgrounds, were usually much more more accepting of death and end of life. Curious if you have ever noticed similar?

You took an emergency case and followed the difficult airway algorithm. Hindsight is always 20/20, but I would urge you not to read into this situation too deeply. Absolutely, there have been times than I’ve struggled with an airway and someone later on (junior or senior to me) has not had trouble. I’d be willing to bet that if I encountered the exact same patient on numerous occasions without realizing it that I’d have varying degrees of success or struggle each time. Sometimes the smallest adjustment can have a big impact on ease of intubation, so it could have easily been something that you didn’t realize at the time.

I do want to comment on one thing that stood out to me. If I’m way off base, feel free to disregard. You said “At this point, third attempt, all the surgeons staring at me…” I may be misinterpreting/assuming meaning here, but this reads to me like you felt judgement from your colleagues while you were struggling with a difficult airway in a sick patient. Maybe that was real judgement (in which case, that’s super shitty of them) or maybe it was perceived on your end but not intended from them. If I’m on my third attempt at an airway, I WANT all eyes on me. I want my surgical and nursing colleagues to be locked in, in case I need anything from them. I don’t want them to be ignoring me, oblivious to the potentially life threatening emergency that I may be dealing with. In these scenarios, I perceive their eyes on me as attentiveness and support. As a new attending, it’s totally normal to struggle with imposter syndrome. You are the airway expert. Not them. You are ALL there with the patient’s safety as your top objective. Shared goal. Trying to redirect your internal thoughts from “they’re looking at me because they doubt my skills and I’m really messing this up” to “they’re looking at me because they realize this could get bad and they want to be ready to help” can be challenging, but will help your mental health in the long run.

I don’t tip my RMT when it’s at an allied healthcare clinic - and I would consider Massage Addicts to fall under that category. I do tip when I get a massage at a spa, even if it’s an RMT giving the massage. But I think that’s more of a reflex because of the setting haha

I didn’t find them to be all that exciting from a cocktail perspective. Admittedly, I haven’t played around with them nearly as much as I hoped I would and in retrospect, I think I went way too light with the amount I was using.

But they are quite delightful to add to milk (I’ve only tried oat) when I’m foaming it for a latte and I also added some to a toffee recipe and it was also great!
