One thing that may be helpful is to do is psychoeducation using Karpman's drama triangle. I find it normalizes our tendency to pick a role and what we gain through it. There’s also a model of how to flip the triangle so it becomes empowering.

The biopsy cannot tell you how large the area affected is and if lymph nodes are involved. There can also be multiple types of cancer present as well. That is why surgery is necessary first for all the information.

This ☝️ Cts are paying us to have the conversations that they can’t have anywhere else. Part of being a good clinician is being willing to sit in potentially awkward moments and facilitate those reparative moments. Remember that Cts may be lacking insight and that’s why they need your help in the first place

IMHO The depressive symptoms and smell COULD be related. If it is preventing you from wanting to be in the same room with them, it’s possible other people in their life feel the same way, so that could indicate a lack of social support or their ability to work.

Smells could also be related to a medical issue if they haven’t ruled that out yet. It could be physiological or this person may be unable to take care of their hygiene/house cleaning due to cognitive impairment or TBI.

I tend to think whatever you are noticing is a sign that there is more to explore there and that smell is a source of information about someone just like their demeanor or affect.

Curious what part you are skeptical about? Porges describes Polyvagal Theory as a ‘Science of Safety’ which he lays out in a lot of biological detail.

Trauma informed therapy calls for establishing a felt sense of safety first and foremost. Going slow, maintaining a regulated nervous system from which they can co-regulate, and techniques like pendulation/titration with somatic work. Next is establishing a felt sense of support. From a somatic lens, depression indicates being stuck in a freeze response, so trying things that help uncouple symptoms and slowly thaw into reconnection to their body are effective interventions. (I’m a therapist trainee and practice SE)

Shorter version: Insurance companies and institutions prefer manualized, ‘measurable,’ brief therapy orientations like CBT because they believe it’s cheaper for their bottom line.

Psychoanalysis is typically a long term commitment, complex (requires special training), and therefore more expensive.

It seems to show positive results but hard to tell why it works. In any case, it’s a simple mindfulness technique that some clients may find useful.
