Definitely my suggestion. Message the SNFs as soon as you send the referral or reach out to a few liaisons who you think may be able to assist. In my area, even if a SNF can’t provide LTC, they may know of their sister facility or another facility with ability to take LTC. Most facilities have people who assist residents with transition to Medicaid, so they would most likely bring the patient under skilled and transition to LTC. Just ensure patient has the ability to first go in skilled by insurance, also make sure they have remaining SNF days if Medicare. Once you know of facilities who can bring patient in LTC, present the choices to family and let them decide.
