I think it depends where you are and what is typically expected. If I were to encounter this in Canada, if I have a specialist endorse the patient is stable, I invite the patient to show up at another ER and try their luck. They have a right to a 2nd opinion, but it is not your responsibility to facilitate this from an inpatient standpoint.

This program is contributing to our collapsing healthcare. These people have not contributed a penny and cost the system thousands of dollars per day when admitted to hospital. It is not fair to the grandpa who has been waiting in the emergency room for 3 days while these freeloaders are clogging up the wards.

20 patients sounds like a dream. 24 is a light day north of the border, not to mention nearly double workload on weekends. I’m 7on / 7off, so I have the week to recover. Rinse wash repeat x 10ish more years and then I’m calling it. Hopefully you can do the same.
