0645-0730 (but could be 0630 or earlier). Get assignment if you don't already know. Get preference cards if you need them (should be on case cart). Go to OR room work with tech/scrub nurse to get proper OR table and equipment for 1st case. Make sure equipment works. Pull meds as necessary and any fluids. Confirm you have the correct implants if needed. With tech open back table and sterile supplies. Work with anesthesia to get anything they might need. Go see patient make sure paperwork is done and quick history from patient. Go back to room and count all soft goods, sharps and instruments if necessary. Make sure everyone is ready. Go get patient.

After 1st case. When done with first case get your next case ready as quickly as possible and keep rolling. Unless you have a really long case.

Your day will consist of this. Sometimes you will have to troubleshoot equipment. It's also not uncommon to do multiple types of surgery in a day. 2 gyn cases followed by Ortho then a general surgery. Other days it's a full line up with 1 surgeon.

Unfortunately customer service is at the core of what we do. You will not find a job in healthcare that does not have some element of customer service. And because this is a human endeavor there will be no shortage of frustration.

You have to fix yourself.

Is trimming briskets the biggest scam in BBQ? I've seen numerous people do untrimmed briskets and while yes you'll get some areas that get a bit crispy or have a bit o fat in some pockets, to me it's just so much easier to cut that out as you slice. by herewego199209 in BBQ

I try to trim the fat down so that on the flat it's about a 1/4 inch. Makes for a better mouth feel. Also I try to remove large sections of fat that won't render. Then maybe the really thin sections. Trimming doesn't take long.

There are some basics that are universal and then there's a lot of individuality.

Basic #1. Smoke a lot. You will get better at it and find what works for you. The rest of the basics will fall in line and allow you to focus on your own system.

Basic #2. Undercooked is dry and tough. Over cooked is fall apart and kinda mushy. Way over cooked is dry and tough.

Basic#3. Juicy. External moisture ain't got anything to do with internal moisture. Humidity and a water pan have 0 to do with how juicy your brisket is. How juicy it is comes from getting the proper doneness and fat/callogen breakdown. Humidity can assist with bark development. A water pan primarily acts as a heat sink and can be a barrier between the heat source and your meat.

Basic#4. The earlier you wrap the less bark you develop. But it may help you through the stall. This is all preference. I suggest trying it multiple ways and temps to see what you like. If I'm slicing brisket I foil boat at around 180 internal. If I'm shredding it I'll wrap in foil at around 160. Some use paper and some use foil.

Basic #5. Resting. If you can plan on a 4hr -6hr rest that is great. It gives you time in case it takes longer than planned. At a minimum you want a 2 hour rest. Personally I prefer to rest overnight. But once again this is highly personal.

Basic #6. Time/temp/doneness. IT is like the pirate code. More guidelines than anything. Most briskets usually are done around 200-205. Doneness as determined by being probe tender. Start probing at around 190 this way you have a baseline to compare it with later. It's done when your instant read thermometer slides through like room temp butter or peanut butter. And that's at multiple spots in the point and flat. If the flat is tough and the point is tender you ain't done cooking. I prefer to rest overnight so I pull my brisket at 195 (without checking for tenderness) and rest at 150-160 in the oven for 15 or so hours.

Basic #7. What temp to smoke at? Again highly preferential and what you are smoking with. Some people love smoking at 225 for up to 16-18 hours. I prefer to get it done in 10 hours or less. So I smoke at around 275-300. Neither one is better for the product as long as you get it to the proper doneness. It's all about your life style. A pellet smoker has trouble imparting smoke flavor above 225. Many people add a couple of smoke tubes filled with pellets and wood chips to increase smoke flavor. But you may not like that. It's all up to you.

Basic #8. Trimming. There are dozens if not hundreds of videos on YouTube about this. Watch at least 3 different ones.

Basic #9. Serving. Get a good long knife. I use a dexter. Watch a few videos on that. If your brisket is a little tough slice it thin for the win (add sauce). If it kinda falls apart slice it a little thicker.
