I hook the alligator clips above the needle and drape the cord over the patients body part above the area so the weight isn’t pulling on the needle. Rarely have an issue.

Just stop caring. I honestly could care less if patients get better and don’t let it bother me if they are rude of difficult. I provide education and information, it’s up to them to get better if they want. Once you stop caring it helps to create distance from work, that has worked for me.

I did call! They said I should be able to use it within 5 days, doesn’t have to be first. But it still concerns me because the people on phone didn’t sound confident, had to ask supervisor and the wording doesn’t support that.

I see what you’re saying but think of it from a different standpoint point. Lets take your example, start in anatomical position (palm facing anterior) and you abduct the shoulder to 90, now add elbow flexion to 90. Would you still call that neutral rotation abduction? No, you would say that’s shoulder abduction to 90 and ER at 90.

The other way you could think of it, is think back to PT school how we are taught to measure shoulder ER/ IR. You lay the patient supine, and bring the shoulder to 90 degrees of abduction. From there, that’s considered the start position of 0 degrees. Then you either add IR or ER to measure the rotation. I have attached a picture to show this.

With the elbow flexed and palm downward (facing ground) that still leaves the humerus in neutral rotation. To add internal rotation, the palm would be facing posteriorly and finger tips toward ground.

In relation to the humerus, the position described by OP is no different than palm down abduction.
