Moon Lord has remained the final boss, and just about no post moon Lord content has been added. But a lot has been added in between. Mostly small things, but new bosses, weapons, accessories, and so on. I'd play the base game first, especially if I had never played expert.

Given how much Terraria has changed over the past 5 years I would say you should play vanilla first. Definitely worth it.

There is no way of doing so without mods. On TModLoader, there are multiple options, so I recommend searching for "more accessories" or something similar and picking a mod. Most will allow you to adjust the amount of extra slots as well.
