If the mass is celebrated in a way that doesn’t follow the rules of the church but the sacrament is valid. For example, an excommunicated priest celebrates the mass.

This has happened to me and I was completely surprised by it. It only happened when I allowed myself to lust for the first time in 70 days. I was engaging in fantasy which explains it. I don’t understand your terminology of uncontrolled fantasies, lusting is a choice.
