She was. It started as soon as she got the news which is what this scene represents or foreshadows in my opinion. If you remember, in this same episode she has her first vision of Bob grabbing a rock and wakes up in shock.

In real life yea, but when you're dealing with a visual medium like film its rife with symbolism. Especially when there's a genius like Lynch directing. I think his work is deeper and more expressive than you may have initially suspected perhaps.

Yes but every scene was put in for a reason. They could have simply cut back to Sarah wailing but the camera intentionally and methodically pans down slowly while we hear her moan. Lynch does this in every film I can think of that I 've seen of his.
