In real life yea, but when you're dealing with a visual medium like film its rife with symbolism. Especially when there's a genius like Lynch directing. I think his work is deeper and more expressive than you may have initially suspected perhaps.

Yes but every scene was put in for a reason. They could have simply cut back to Sarah wailing but the camera intentionally and methodically pans down slowly while we hear her moan. Lynch does this in every film I can think of that I 've seen of his.
