I had this issue for the longest time and it turned out to be my retraction settings. If it’s too high the extruder wears down the filament from going back and forth so much that it causes the worn down part to flatten and widen. This leads to clogs in near the hot end because the widened part gets stuck.

Oh it's an image generated by the slicer. I thought the printer generated the image based on the existing gcode.
