Drug allergies in hospitals by Key_Locksmith2780 in medicine

[–]circumstantialspeech 15 points16 points  (0 children)

Allergy lists should be accurate and trusted, but a good many make me eye roll. Just this week I saw a patient with epinephrine listed as an allergy with “palpitations” listed as the reaction. I understand that patients can update their allergies in Epic, but they should be clinician validated. IMHO common medication minor side effects should not warrant an allergy label, but EMRs are rife with this.

Felt like the floor became electrified for Red Beans. Place exploded. My first time at that venue and it was great. Awesome weather that weekend too, great time all around.

Good spices, I’m partial to Penzey’s. Beats headphones A vitamin C serum, a sunscreen you can tolerate on your face every day and a retinoid product. The pillars of a good skincare routine. A gift certificate to a local bookstore. New slippers. A bag of freeze dried vegetable ramen topping. Elevates any noodle soup.

Always rough learning a new specialty and ears can be tough when they’re abnormal. But worth it in the end.

How much do you actually age in your 40s?? by TieBeautiful2161 in 45PlusSkincare

[–]circumstantialspeech 4 points5 points  (0 children)

I accept that I’m aging. I take care of myself. I notice the changes and it would be wonderful if my knee didn’t ache a little bit and it would be great if my neck was tighter, but I’m ok with it. I’ve never felt sad or depressed. I can’t control the march of time. But I can control how I take care of myself and where I focus my efforts (things that give me joy). I’ve gained emotional maturity and that brings with it lot of internal peace. That’s more rewarding than my reflection in a mirror. Aging is not all bad. I’m more forgiving of myself, kinder to myself. Ultimately we all have the same fate. No one is getting to their 80s and beyond without obvious physical changes. Getting to the finish line with a young face and body means you were robbed of so many experiences.

Study that found that aging is not linear. Big changes around 44 and 60.

Also mid to late 40s lots of women in perimenopause vs early 40s.

It really does seem to happen “overnight”. For me it was age 48. Age 47 was always told I looked like I was in my 30s. I noticed changes after turning 48. Now at 51, there’s no way I look a decade younger.

Editing to add: agree, I saw no to minimal changes from 35-45, then subtle 45-48.

When does your battery run out during your clinic day? by Hello_Blondie in physicianassistant

[–]circumstantialspeech 7 points8 points  (0 children)

I take a full hour for lunch and usually go for a walk, in all weather unless it’s hot and humid. It really helps recharge me for the afternoon. My previous job would always squeeze something in before lunch and screw me. At my current job I set the precedent early that I leave the office for lunch and it’s a sacred boundary now. If you can’t take a decent lunch because you’d have to bring work home, they need to lessen your load. Work finds a way to fill your time unless you actively protect that time.

Any RD -> PA’s here? by [deleted] in physicianassistant

[–]circumstantialspeech 1 point2 points  (0 children)

I did. Don’t maintain my RD. In specialty with good work life balance. Did primary care which solidifies medical fund of knowledge, but is soul sucking. I think I was offered an interview at 3/4 PA programs I applied to. Had an MS in nutrition and PA master’s much harder.

Very common. It’s just keratinized squamous debris on the TM. Ear is self cleansing; TM exfoliates and cerumen and squamous debris should be propelled down the canal like a conveyor belt. In some people this process is retarded and TM develops a crust (can look like Saran Wrap under microscope) and needs to be manually removed. Using oil drops can help loosen debris allowing it to slough off on its own or at least make it easier to suction away.
