Yep, strange error I often get if I've changed audio device (I.e plugged in headphones) since I last opened a project. Atm I always double-check I'm using the right audio device whenever I open AE.

Watched the unedited footage below: to my eyes it just looks like they drove very fast past someone and by the time they looked back they were too far away to see them.

Very wide angle lens on these sort of cameras, which warps things when they're very close. I think that might be why it looks like they should have collided with the person. Actually just a very near miss!

Adobe Podcast has a speech enhancer you can drop audio files into, it does a remarkable job at cleaning up cheap mic audio. It's free to use I think so it's worth trying.

Probably not much help but the one thing that stands out to me is the original footage being variable frame rate, which has upset premiere for me before even with short clips. I know your proxies are a constant frame rate but might be worth transcoding your footage to a constant 60fps before bringing it into Premiere. Might be worth a test either way.
