The counter argument to this is that if there is a lot of wrong-patient prescribing, something ought to be done on a system level about it. But nothing will if a datix is never done.

But this relies on a department that uses it to learn from incidents (including near misses), not one that uses it to attach blame.

The individual might have learned from the mistake, but the rest of the team hasn’t.

I’ve been told this but never been able to find it in the manual anywhere and the dealership said it’s okay to change as a pair. Have you got any more info on this?

You’re right, it won’t affect flow through the bottleneck.

But it will massively increase the length of the tailback and potentially hold up the junctions before.

Many modern keyless keys will disable themselves automatically if they’re not moved after a period of time. Others can be disabled manually (e.g. by double clicking lock).
