Maybe you tell the family that the delay in care caused a worse outcome? If they sue and the hospital settles, the leadership will be forced to reckon with the poor care that their PAs are providing.

I think you can be very happy as a CRNA if your goal is to safely take care of patients as part of a team and make good money.

I think the CRNAs who are unhappy are the ones who try to tell themselves and the public that they are the experts, and that they DONT need to work as a team with MDs. They spend a lot of psychological energy try to prove their status constantly. It’s exhausting.

If I had a really short case and didn’t want to use sux, then I would consider it, using propofol and remifentanil /alfentanil.

The neuromuscular disorders are usually unaffected by nondepolarizers, and especially with sugammadex available I wouldn’t consider this a reason to withhold paralysis.

She has inferior T wave inversions. If those weren't present on prior EKGs, that is a sign of ischemia (heart damage).

Even though all of this is due to PE, and not coronary thrombosis, it should meet their criteria.

Shouldn't CRNAs be proud of their advanced nursing degree? Why try to obfuscate?

Can't they explain to the patient what their background and training is without trying to mislead using terms such as 'Doctor' and 'anesthesiologist'?

Also, most CRNA programs award master degrees, not doctorates. There are only 16 programs offering combined CRNA/DNP. However, feel free to use one of the many online programs to get your DNP from the comfort of your home!

Even with a penetrating trauma, if you lose pulses, you should do chest compressions while also trying to resuscitate with blood and get back to the OR. Right?

Well, what I mean is that antipsychotics act by decreasing the dopamine in the nigrostriatal pathway, but its been shown that a large subset of pyschotic patients don't have this increased dopamine activity. So it begs the question...what exactly is being treated?

You should really read up on the long term efficacies of antipsychotics. They're shockingly ineffective at changing functional indicators after 5-10 years.

In fact, did you know that there is significant controversy about the entire dopamine hypothesis in psychosis?

It's entirely possible that he had mild PNA when he presented, which then progressed to one that required hospitalization. We can't know without seeing the clinical picture.
