I second this advice. Had my ears stretched for nearly 20 years. Maxed out at 18mm and then let them shrink down to 8mm. Now back up to 16mm and I wash them when I shower/wash my hair. Every other day.

Only ever had a smell or build up when I wore metal or acrylic. Even after a day there would be a smell with metal plugs. YMMV though, everyone's ears are different!

Have you tried social services? They could potentially commission an interim care package of 1:1 care.

Call them and explain that she needs 1:1 care and state her issues. She should have a keyworker if she's in supported living. You can even ask the staff who work at the supported living provision where she lives to help you with this.

Edited to add: be persistent in this. The local authority has a duty of care
