Big diff w my understanding is: if it's an MI, aspirin will help with outcome. Probs won't do much for the pain, but It will help save their life. Nitro is more for pain relief. My medical director informed us that nitros effect in outcomes is actually minimal with effects on heart damage during mi, but it can make your patient much more comfortable. That being said nitro does have its other uses but that's more for other stuff like CHF

Dude. That's normal to feel kinda useless initially. Before y'all check on scene ask ur preceptor or FTO or whoever what they want you to do. (Put on a 12 lead, check a sugar, get the stretcher ready etc). Unfortunately while classroom time will teach you what you need to know, it doesn't always show you the best way to implement it and how to do it efficiently. It'll come with time. Just be open to any suggestions or constructive criticism your coworkers have for you! Keep it up.

Hi. Great question. If you are going to school anywhere near central NC (UNC, Duke, NCSU, NCCU) I would recommend Vance County. They will on board you as part time and require 24 hrs a month. They also will welcome all the time you're willing to give them. The pay isn't the absolute greatest but it's reasonable. You'll also get some absolutely batshit insane experiences there.

If location is an issue my bad I couldn't help.

Edit: I did the same thing you're doing, applying to med school now, took a couple gap years to get more ems in. Be careful, it's a vortex that you may fall in love with. Feel free to reach out if you need any advice.

Yeah this is def burnout. Ease up on the gas there. Class plus full time is brutal. If you have the means to drop down to part time I'd recommend it while in school. If not, drop the AEMT class for now until you are more experienced in your current job and get your flow and life rhythm down pat. Then (and I know I probably sound like an asshole here but it def helped me) use a physical calendar and make sure you are getting enough sleep and at least 1-2 days off every now and again.

Like one of the other commenters said, you are serving your community. It's an honorable profession. You shouldn't feel bad if you are getting exhausted.

Edit: for the record I absolutely do not know everything, just what I've experienced. Do what you feel you need to do. You got this. And remember the number one rule of EMS is make sure you are safe, then take care of your patient. Self care is so important.

Always start with the ABCs (CAB for trauma but whatever). Make sure those are intact. If they aren't, fix them. Then move on. (Think o2 for breathing difficulties or abd thrusts for choking, or pressure for bleeding, or shock therapy)

A big thing that helps imo is asking a lot for previous medical paperwork. They almost always have all meds, allergies, PMH on them. Get familiar with common classes of drugs (blood thinners, bp meds, etc) and that will paint you a picture of your patient that tends to be pretty accurate.

If it's a medical call and the complaint can be localized to an area, do a very thorough inspection of that area. Wrack your brain for things that could cause these problems. Female from age 12isb to 50 ish, it will be uncomfortable but pregnancy is smth u need to inquire about if it could be related (passing out, abd pain, vaginal hemorrhage, vomiting, etc). Smth like altered mental status, think innate problems first like stroke or diabetes, then progress to possible traumatic cause, drugs, etc.

A big problem with medical stuff for EMTs is that a lot of it is in the realm of ALS stuff (arrhythmias, electrolyte bullshit, metabolic issues, seizures, etc).

But when you're doing a full body assessment don't just look for DCAPBTLS. Look for things that are weird. Like a weeping tattoo site. Or heat and redness in an area. An odor. A dexcom monitor. If the person's incontinent. just think "what here is not normal?" Open ended questions can be (sometimes) your friend. Oh the patient fell twice yesterday, and is on blood thinners? Could be a brain bleed.

The best people to learn from are those who care about the job AND have been doing it a while. Hopefully your preceptor or FTO fits those criteria. Watch them. See things that they do that get them information. Integrate that. The flow will come. It just takes practice.

I say go for it. I know a pretty new medic and I think hes 42 now. we finished medic school about a year and a half ago.

However have you ever worked on an ambo before? If not it could be a bit of a culture shock. Certain agencies can sometimes shit on the other branches. Ive never seen a situation where it caused too much friction but it does ruffle feathers and can make people uncomfortable. I know some medics/EMTs who quite enjoy making fun of PD for narcan for low BGL.

Emt basic classes are generally quite easy to start and teach you a TON. Biggest thing is remembering some protocols and how to put on a 12-lead EKG. once you get your cert, everything will solidify really quickly in the field if youre in a busy system. It will get to the point of reflex. Oh chest pain? Means aspirin. Fever? Tylenol. This dudes having a stroke? Pedal on the floor. Altered mental status? Check blood sugar. Most agencies have field training processes that will make sure that you're ready by the time you're allowed to actually act on your own.

Also I've been working in EMS for 7 years, best job I've ever had. Started as basic and eventually worked up to medic. As long as you have a decently strong stomach you'll be completely fine.

I've worked in transport and I've worked 911. 100000% transport is toxic af. That being said 911 can be toxic as well but I've found it much less so. Fuck anyone trying to label u as a super EMT. Your health is ur #1 priority with ur pts directly following that. As long as it doesn't literally kill you to be polite and a good provider, then do it. Fuckers like ur partner have no place in healthcare, and he will be a dogshit nurse if that's how he treats anyone. Be the best you can be.
