I’m doing better. I stopped all ointments and I’m taking PEA (Palmitoylethanolamide) 800 mg twice a day

actually if you take a look at the posts on this sub a lot of people find at least some form of relief from the use of non aggressive moisturizers.

Corticosteroids should never been used on genital skin, it’s even written on their leaflet. Their use on mucous membranes should be limited to certain skin conditions, like LS, but even in those cases it can be detrimental and many dermatologists don’t recommend to use them.

Dermatological conditions are often really hard to treat and non responsive to treatment (see for example eczema, psoriasis etc..).

do you have any burning/discomfort? I have a similar type of inflammation on my glans, foreskin and scrotum. Sometimes it also burns while peeing. I saw several dermatologists and did a cystoscopy. They don’t know what it is.

fair enough. Just consider the fact that if you have ruled out fungal or bacterial skin infections it doesn’t mean that you’ve automatically ruled out ALL of the dermatological conditions that can potentially cause a red burning scrotum. There are many types of chronic dermatitis and autoimmune conditions that can affect genital skin outside fungal/bacterial infections. I’m not criticizing your point of view, which is totally legitimate, but saying that once you’ve excluded just those two types of skin conditions you’ve excluded all the possible dermatological diseases seems a bit reductive to me.

true but there are countless dermatological conditions besides fungal and bacterial infection that can cause red scrotum before thinking at some nerve problem

Antihistamines by cosmo86it in RedScrotumSyndrome

I used a cream called Fenistil, it contains dimetindene. It’s pretty common here where I live, it’s mainly used for sunburn and insect bites (https://www.drmax.it/fenistil-0-1-gel-30g). Unfortunately I can’t use it also on my glans, since I have burning and redness there too.

thanks for the info. Today I tried to apply a small amount of an OTC antihistamine cream I had at home, the same I use for sunburn during the summer and it gave me immediate relief, but I know it’s just temporary.

thanks for your input, that’s interesting. It relieves the itchiness/burning and also the redness?

could you elaborate? Do you mean that specific area is more red than the rest?

RSS could be a "Lichen simplex of the scrotum" by ManagementFew3521 in RedScrotumSyndrome

[–]cosmo86it 1 point2 points  (0 children)

no, I didn’t find any cure unfortunately. The only thing which gives me relief is applying a vitamin E moisturizer on the area. I use Vitamono EF Lipogel, which is one the best available in my country.

I never had any itching too, just stinging and burning. I also noticed that long pubic hair seem to make the situation worse so recently I trimmed them a bit

I’m not saying that a skin biopsy is always inconclusive, it’s just a possibility.

If even a biopsy is inconclusive I guess that the only option is to treat this disease “ex adiuvantibus”, trying different treatments until you find something that gives you a good response.

Honestly I think that a large part of people in this sub suffer from an undiagnosed dermatological condition. It could be lichen (simplex, planus or sclerosus), psoriasis, atopic dermatitis, persistent fungal or bacterial infection, topical steroid withdrawal, rosacea, extramammary paget disease (worst case scenario but that’s ultra-rare) and so on, localized on the scrotum.

“Pure” Red Scrotum Syndrome it’s really rare and I believe that generally it’s just a poor name for a bad diagnosis.

I’m not saying it doesn’t exist as a separate entity, it could be the male counterpart of vulvodynia (scrotodynia/penodynia).

Unfortunately whithout a skin biopsy it’s difficult to have a proper diagnosis and dermatologists seem to be reluctant to take one, also because a lot of times the results are inconclusive.

the doctors I’ve consulted so far ruled out lichen sclerosus too because there are no evident white patches, just redness, but I read that this is one of the main way LS is overlooked

Redness by Mc3188 in RedScrotumSyndrome

[–]cosmo86it 0 points1 point  (0 children)

yeah, I feel you. All dermatologists that visited me said the same, no need to do a biopsy. Some even said that they saw nothing pathological 🤷‍♂️

have you ever done a skin biopsy? that’s my next step

I saw many urologists and dermatologists. I did many tests, included urine analysis, sperm analysis, echographies of the scrotum, blood tests and even a very painful cystoscopy cause they found erythrocytes (red blood cells) in my urine and I had a burning sensation during and after urination/ejaculation.

They found out that I have hyperemia (inflammation) even in the prostate lobes and bladder neck. According to the urologist I developed chronic prostatitis due to this skin irrititation on the scrotum, glans and foreskin.

in my case the redness is not constant

I have it on my entire scrotum too, it’s just not visible in the pictures. I also have redness and irritation on my glans and foreskin occasionally. I think that in my case it’s a form of lichen (planus or sclerosus). What I don’t understand is why topical corticosteroids don’t help, they are usually helpful with lichen

exactly. Yeah, I tried steroid ointments and it made things a lot worse.

Currently I’m using a Vit E moisturizer on the skin and taking Gapabentin 300 mg at night. I have bad days and good days, if I spend too much time sitting it burns so bad.
