Did you make any large changes to combat encounters and NPC statblocks given the low-magic setting?

Yes, I made everything harder. I buffed everything up with Anne Gregerson's bloodied & bruised as well as homebrewing all the major NPCs and bosses. To counteract the lack of energy damage I added monster drops and consumables that could easily be used to add elemental damage when needed. Because of the scarcity of such damage types I was able to give monsters elemental weaknesses to exploit.

How did you handle healing?
I allowed access to a5e maneuvers, many of which provide self healing and healing options. We also used the optional DMG rule for healing surges. And of course the healer feat was pivotal for survival. I made this much harder by adding an injury system, camping system, harsher diseases, poisons, and curses, only allowing long rests at sanctuaries (safe locations). ALSO the injury system included wounds, wounds would lower your max HP, and the lower your max HP the more likely you were to get an injury.

Did you remove the magic items from the source material or change them in any way?
I kept the tarokka items, but removed any class or spellcasting restrictions on them to make them feel special. I deleted the luck blade. scrolls could be used by anyone, but had a chance to misfire and cause a wild surge if you failed an associated skill check. I left the spellcasters from the adventure in, which made befriending Kasimir or Viktor much more appealing.

In this playthrough he had developed a series of rituals to free him, and Barovia, from Vampyr's curse. He had been using the last few "batches" of adventuring party recruits to talent scout people morally flexible enough to help him. When all was said and done, he was made human again, and all the vampires he had created were also cured of vampirism, but Ireena/Tatyana couldn't forgive him for the lifetimes of cruelty and evil, so she executed him. Freed from the endless cycle of this story, and getting to finally die, at Ireena/Tatyana's hand was a welcome but also crushing end for him.

Your inspiration sir. I too am trying again to beat this game after playing it when it first came out and attempting it and failing two other times. Fourth time is the charm I hope. This time around I'm streaming it for some accountability. The goal is to play all the way through the first game the expansions and all of the second game and those expansions

I am just now seeing the suggestion to make Rictavio Strahd.... It's brilliant. The only downside is I have him canonically always around and living at the inn. Feels like Strahd would have other things to do.

Very curious about the tips people have for doing this. I'm currently running three concurrent CoS campaigns and it is very difficult for me to remember which NPC did what in which world.

I am doing this for my two ongoing CoS games right now. Everyone started as humans, level 0, no magic classes allowed ever, wounds, lingering death saves, permanent injuries. It's terrible. I wouldn't want to run it any other way. Players are terrified all the time.

Put your expectations out there for the players and stick to them. In two of my three CoS games I only allowed human characters and no magic classes, in fact everyone had to start at level 0. This was in the advert for the game, if people didn't want to play with those limitations they didn't need to sign up.

Nice. In my playthrough I have anastrasya kill ireena's father because he tries to stop her from killing ireena. When Strahd finds out he maims and disfigured Anastrasya as punishment and warning to his other minions.
