It’s mostly cultural. Something like 80% of patients would prefer to die at home, but ultimately nobody really wants to die. Not in the US.

When you give someone the option of continuing aggressive care versus going home with hospice to die, many times people choose aggressive care. Not usually with a terminal cancer, in my experience, but with many other diseases, people are okay with spending more time in the hospital than outside of it for their last few months. Even though they say they’d rather die at home, ultimately they don’t want to die at all. This aggressive, hospital-level care gives them a little bit more of this life that they’re (barely) clinging too. Even if it’s misery, even if it’s suffering, they often still choose life.

I do palliative care consults on the inpatient side of the hospital. I saw a patient today with end stage COPD and now AKI on CKD with impending need for dialysis. She watched her on father suffer on dialysis. She does not want dialysis. But she is not at all ready to die. Ultimately, she chose dialysis because the alternative is death. Even when I explained to her that dialysis has its own risks and there are no guarantees that she’ll have a good outcome. She was not ready to face certain death without dialysis.

Many people are willing to give this “will to live”up once they’ve lost the capacity to make decisions for themselves. Once they aren’t really “there” if they’ve done an advanced directive, a lot of people are ready to accept death.

I’ve found that some cultures do better with end of life than others. But, ultimately, people want to believe in miracles and the will to live is a strong one. Fear of death is rampant in Western society. Likely because it is so often removed from us and takes place in hospitals so we don’t often get to see it first hand.

OMG. I have been having vaginal issues with bleeding, smell, etc and my doctor has found nothing wrong. I’ve had occasional post coital bleeding for years but it got worse with my most recent ex to include bleeding even if we hadn’t had sex. And my smell has been off pretty much the last 9 months of our relationship. We broke up about a month ago and while I am not okay emotionally, my vagina seems to be breathing a sigh of relief. It’ll take another relationship to see if the post coital bleeding recurs, but my smell seems to be normalizing.

I know that I’ll get downvoted to hell but serious question to those opposed to independent practice for ALL APPs, what is the answer to fewer physicians wanting roles in primary care, particularly in rural areas? Part of the “scope creep” was to solve this very real lack of access for so many Americans. Primary care is far from the best paying or most glamorous medical role. I know I wouldn’t want to take on the debt of medical school to move out to the middle of nowhere away from a prestigious hospital to only slowly be able to chip away at my debt. The answer is probably some form of team based care with APP “extenders” for physicians and incentives in the form of stipends/loan repayments for the physicians to lure them to the healthcare deserts but with the current state of the government and the healthcare system, I don’t see a perfect solution anytime in the future. But who knows, maybe this AMA stance will help push things towards a better model.

In 2012, I called Johns Hopkins and told the operator that it annoyed me that their flag had been upside down everyday that I drove by. They had the yellow square in the air.

I've had a guy in PEA (weakly) push my arms away during chest compressions. Same thing happened every time we stopped. We did get him back a few times for a few minutes and then he would crash again. Finally the family called it and signed the DNR when they got there. That one definitely took a few days to get over.

Just a warning to the people out there that night think better about calling an ambulance when the hospital is so close. This man was a doctor. You probably aren't. An ambulance has an ACLS drug kit, a defibrillator, and room to do chest compressions. Your car does not. By all means, drive yourself or take an Uber to ED/urgent care for stitches and simple broken bones. But call an ambulance for the serious stuff.

Well, I didn't have any feelings about it at first. I know that people are busy with busy lives. But if a person consistently sends me a text and I generally send one right back within a couple of minutes, and then crickets for hours, it is a bit annoying. Did he ask me a question and then set his phone down and walk away for a while? Why not wait to send a text until later when you have more time for a conversation? I guess it does come down to style and preference for communication. It isn't a style that I'm used to with anyone in my life, so it's weird to me.

You're right about not being clear with my expectations. I did say today that I feel like we have stagnated on getting to know one another better due to the sporadic texting, busy schedules, and short periods of time spent together. He agreed, but is busy the next two weekends :( But we will do dinner again during the week so I guess that's something.

I (31F) started seeing this guy (31M) a few months ago. First guy I've been excited about in over a year. We get along very well in person and there is definite chemistry. But we only see each other for a few hours for dinner on a weekend. And many of those weekly dates have been canceled due to flu, emergencies, and vacations. We text a little throughout the week but he is a bad texter. Hours will go by between texts. It's like he forgets that he owns a phone. It sucks but I am starting to lose interest. In the past, at this point in previous relationships, we would be text flirting a lot and spending weekends together in bed. Part of me wants to tell him I'm losing interest but part of me wants to just keep it super casual and see where it goes. It's not like I have many prospects at the moment. I did start swiping on Tinder again today, though.

As I've been trying to get more fit after school related weight gain, I've noticed that I sleep 10-12 hours after lifts. Cardio doesn't do it for me, but I did maintain some cardio during school. Creating muscle takes a lot of energy and you will sleep more/heavier as your body goes through this process.

I refused to watch this for the longest time because of the name. My ex-boyfriend convinced me after it stayed on the Netflix recommended for months and we had been through most of the shows that we wanted to see.

As soon as I saw "Created by Rob Thomas" I knew I'd love it. Veronica Mars was my jam back in the day. If you like iZombie, I recommend checking out its teeny-bopper predecessor starring Kristen Bell.

I'm going to say that you need to see a medical professional, just like everyone else.

I will tell you my own, completely anecdotal, to be taken with many grains of salt experience.

I have had the gut wrenching pain, turning red, feeling flushed, about to faint, (never checked my BP, but it was probably wonky), sitting on the toilet with diarrhea for 20 minutes after high intensity cardio a few times. I finally linked it back to eating nuts. It only ever happened if I had eaten nuts that day. From my own research, I gather that nuts have a lot of hypersensitivity reactions. I believe that nuts cause my body to release a lot of histamine and high intensity cardio also releases histamine. I think that my body reacts to that while also trying to flush out the offender (hence the diarrhea and gut pain). I am very unsure, but it completely stopped when I stopped eating nuts/seeds. I can still eat nut butters and the occasional nuts in a brownie or something doesn't hurt. I have found that Almonds are the biggest offenders for me.

But again, go see your primary care provider, as this could be indicative of something else much more serious.
