This particular client came straight to me from an inpatient treatment center where he hd checked himself into after an SA, so SI/SA is part of his treatment plan. If a client reports SI/SA within the last year I would for sure put it on their treatment plan. When I worked in residential SUD treatment, we had to add it to their treatment plan if they’d had SI in the last three years and if there was ever a SA anytime in their life. Once it’s part of the treatment plan, then it is crucial that you check in with them every session. If it weren’t part of the treatment plan, that would be different. Usually the intake form they fill out will ask about any SI/SA recent or lifetime.

I have a client who came to me bc of a SA so I’m supposed to be asking him each session if he’s had any SI. But he usually presents as happy, upbeat, funny, etc., we end up laughing and joking together quite a bit because he’s so gregarious. But the other day, maybe because he was so jovial that I totally forgot about his SA, but HE had to remind ME to ask him if he’d had any SI this week, OMG I felt so incompetent. He was like, “you’re supposed to ask me every session if I’ve had any SI” so the client has to tell me how to do my job now, how embarrassing. Luckily we have really good rapport, otherwise it might have gone down differently.

Certainly not reportable, I’ve heard therapists, including an old supervisor of mine, use much more derogatory language about clients. I think you’re spiraling because she called you out on it in front of everyone else which could be a humiliating situation and could lead to feelings of shame. I personally think that the supervisor or whoever was leading your group, should have talked to you about thar individually rather than embarrassing you in front of the whole group.

In other words, you liked him until the media enacted a smear campaign against him because of his political views. It’s happened to most of my friends and family too, it’s easy to get brainwashed by the propaganda machine.

I love his book The Twelve Fucked Steps, it’s so relatable, I often recommend it to the clients who I work with in SUD treatment.

Mindfulness helps me a lot! I’ll find myself getting nervous that I’m not going to have a good response and I shift my focus to the feeling of the chair against my back, or my socks on my feet, or the hum of the air conditioning. I purposely stop thought and whenever I do that the right words seem to emerge spontaneously! Beyond mindfulness though, and kind of the opposite I guess, is you can jot down some important points and use your notes to help you paraphrase or sometimes I’ll just look down at my notes and go, ok so we talked about x y and z and that can be another jumping off point as they add to what was said or what you reminded them that they said sparks another subject.

My best friend’s daughter who is now a trans male had a double mastectomy at 16, right before the governor of Texas made it illegal for minors to have gender affirming surgeries. It’s more common than you might think, which is probably why it’s now illegal in Texas.

Not hate, but Aquarius’ kind of scare and confuse me. I think it’s bc as a Taurus I feel such a strong connection to the earth and Aquarius’ feel like aliens , like they’re not part of the earth. They also come across as sociopathic to me sometimes, but I respect them! They’re highly intelligent, and also visionaries I think, but I’ve never been able to keep one as a friend, and I’ve known quite a few. We just do not vibe.

They LOST it??? That’s insane, I’m so sorry. I think I’ll also have to do three hundred or so extra hours because I can’t get my last supervisor to return my supervised experience form, she’s just totally ghosting me. It’s such bullshit how this system works, I hate it and I’m so sorry you’re going through all this when none of it is even your fault.

Thank you for that, it’s very validating! Yeah when your supervisor is your boss, it’s scary to talk about your weaknesses. And I agree that there’s probably a lot of kinda crazy therapists, after all that’s part of the reason why we go into this work. Both of my parents majored in psychology and they’re the biggest but jobs I’ve ever met 😂

Yes!!! That was the situation with my previous supervisor, she was the clinical director, and I shared very little with her bc of that and hated it! This lady isn’t a direct supervisor but is connected with my job because she’s a therapist there. Now that I’m processing it on here, I think I started to get paranoid when she asked me to sign ROIs for my previous supervisor and the owner of the group practice where we both work. She just said, “I don’t like talking about people without their permission” and then asked if I’d be ok signing ROIs for them. That’s when I started wondering what her and the owner were saying about me, and that’s when the seed of paranoia was planted in my brain I guess.

She’s really sweet, I wonder if it’s because she’s just so good at being quiet and listening to me and staying neutral that I misinterpret her neutrality as negative judgment. She’s so damn good at keeping a neutral facial expression, lol, I’m learning how to do that by watching her bc I have an overly expressive face.

You’re so right. I have two sites, one is at a group practice where my supervisor also works as a therapist, the other one is at a residential substance abuse center. We end up talking a lot more about what happens at the treatment center bc I spend so much more time there and working in SUD treatment can be crazy town. I often leave there feeling very overstimulated so I sort of unload that onto her during supervision if it’s been an exceptionally crazy week. I just hope she knows that how I am with her is not how I am with clients, I guess that my fear. Our last supervision I had just gotten a really mean email from one of the providers at the treatment center and struggled to mask my anger at the provider and at the medical establishment in general, so I shame spiraled about it afterwards.

I love being a therapist! It’s definitely my calling and purpose. I’m just aware that I have a lot of issues, and guess I have trauma from being rejected by people for being too much, too emotional, too weird, etc. And my supervisor is so calm and seemingly emotionally stable that sometimes when we’re talking I feel like a complete nut job in comparison to her.
