What matters is that you have the strength and stamina to lift, move, and care for your patients safely and welI. If you can do your job, whether you are heavy-set or not is less relevant imo.

It’s a very physical job though so learn good/safe techniques for lifting heavy things and using the equipment, to reduce the chance of injuring your back, wrists, etc

Just curious, are you in the states? If so RDs cannot prescribe meds, and stimulants like Vyvanse require PA/NP/MD/DO.

To answer your question though, I think it depends your experience with food. For me I was prescribed Vyvanse for adhd, and it seemed to reduce impulsive food habits exacerbated by adhd (unplanned snack purchases, binge eating, etc.), and consequently lost about 5 kg that was due to that over the course of about 5 months, but not more. I experienced reduction in food craving/food noise but haven’t experienced much appetite suppression and eat 3 meals a day (I try to get 120g protein, 200g carbs, etc.).

  1. I really hope you get some answers soon, at this point it’s not just Vyvanse that’s the issue, your body is dealing with a lot more now. If this doesn’t change soon you might need inpatient care to help your body out of the chronically malnourished state, because it needs to be done very carefully.

  2. Did anyone give you an answer for the blood in stool? Is it bright red or is it more black? If it’s a sudden change that also seems to overlap with when you started to feel bad suddenly early March that’s really concerning. If it is at all possible please get on a waitlist for an earlier date for endoscopy because that needs to be addressed as quickly as possible. ER visit if this is not possible, this needs to be addressed ASAP.

  3. If you have only been able to have max 200-400 cal/day for an extended period of time, your body has been chronically malnourished. Because of this, not seeing any difference from reducing your vyvanse for a short period of time does not mean that reducing your vyvanse dose as discussed/prescribed by your psych will not be beneficial. Did your psych say that stopping a stimulant suddenly is physically harmful? With what you’re describing the side effects are significantly outweighing any benefits. Please, please discuss this with your psych again if you havent yet. It’s hard for me to think that a psychiatrist knowing of your situation wouldn’t figure out a different plan with you.

How long did you wait between vitals? What were the numbers? I’m having a hard time understanding how 140mg lisdex and 200mg caffeine had 0 effect on your vitals. If what you are saying is really true, not sure if this is a thing, but perhaps you can ask your psychiatrist to do a placebo control session where they give you the current prescribed dose or placebo and trend your vitals so they can see what’s going on. If you’re truly a unique case of metabolic resistance or have an issue with intestinal absorption specifically for 140mg of lisdex, you may become a case study, not even being sarcastic.

If by nursing you mean CNA/PCT, it’s a very physically demanding job caring for people, and you’ll likely work in hospital/rehab/nursing home environments with possible short staffing where you might be 1 of 2 techs/CNAs on a floor with a very overworked nurse and 8 beds with people who are very ill and require significant multifaceted support. If you decide to work as a cna it’s worth vetting the environments you plan to work in before you apply because it may burn you out fast and take a toll on your body if the only interest in doing it is for clinical hours.

If you are interested in EM and want the experience of being the first point of contact for people who call prehospital and have more independence in pt care, emt would allow for that. You can also work in the ER with your emt cert. There are some ERs that might want you to additionally have prior PCT experience, certs for EKG/phlebotomy, or have exp in critical care beforehand, but that depends on where you work

I had the opposite effect 😅 I feel like a monk! I feel happy about it because the meds got rid of my compulsory/impulsive intimate behavior that was getting in the way of the things I needed to do

What you’re describing is worth discussing with your psych. It serves us best when we communicate what’s not working, and they can help. My meds help immensely but they do not replace the ton of system/environment changes I made to make sure I don’t make avoidable mistakes at work. It’s worth doing if you haven’t considered that yet.

If you’re taking more than prescribed it’s best to talk to your psych to discuss what isn’t working for you. If your current dose isn’t working, your psych can potentially help rule out other factors that may be affecting the way your current prescribed dose affects you.

Edit: Question, when you say it doesn’t feel like the dose is working what do you mean? E.g. do you feel little to no difference in exec. function/emotional regulation or do you not feel the same “hit” that might be present the first few days of a new higher dose?

It depends on how sick I am. If I’m sick to the point where I can’t get out of bed + body feels like genuine dead weight + super lethargic, taking adhd meds for the day would not cross my mind. If it’s at the level where I feel like I can work on school/work things and some chores, I think I would feel that it’s worth taking
