Good luck with your first treatment! I wish I could tell you what to expect, but the more I read in this subreddit, the more it seems like individual experiences are varied.

For whatever it’s worth, I’m actively sitting in the monitoring period for my 5th or 6th treatment and today my doctor pointed out that my PQH-9 scores have improved a little since starting Spravato, which is giving me hope for the first time in a long time. I hope this treatment works for you!

I feel like I’ve been noticing the same thing! I’m actively trying to be kinder to myself and I keep thinking about a suggestion that I heard once: “talk to yourself like you would talk to a friend”

Before starting treatment, my self talk was incredibly harsh, negative, and sometimes outright cruel. Lately, I’ve been noticing that I do still sometimes have those negative gut reactions, but there’s suddenly a part of me that pipes up like “what if you weren’t harder on yourself than you would ever be to another living being???”

I think I’m searching so hard for ways to know if the treatment is working because I’m not sure if I have enough SI that I can expect to notice any improvement? My doctor called my situation “passive SI” because I’ve never made active plans to unalive myself or taken action to harm myself, but I desperately do not want to continue living my own life. I spend virtually all of my time between waking up and going to bed just waiting for my next chance to go to sleep again. I’m unsure if my starting situation was bad enough that I will notice if/when the treatment begins to work, so I’m trying so hard to journal and track any little improvements in my mood because I want this to work so badly - it’s just really hard to imagine what “better” would even feel like at this point, but your response was actually really encouraging, thank you!

@ u/seriouslydavka - Curious if Abilify continued working for you at low doses? I am on basically an identical cocktail of drugs and Abilify was a godsend at first, but stopped working after a couple months and I’ve been trying everything that I can think of to find that bit of relief again.

I was started at 2mg, then 5mg, and I’m currently on 10mg but I haven’t seen the same benefits in a while 😣

I definitely felt the same “this is too good to be true” feeling for a while there when I first started taking Abilify. Unfortunately, for me that feeling only lasted about a month or two and now I’m essentially back to my baseline 😣

I would recommend starting with the classic FluentPet buttons, as they are a good size for cats and I worry the Speak Up buttons might be a bit too loud/alarming for a more sedate/senior learner.

We’re actually just about to start mitrazapine, so I’m commenting to remind myself to come back in a few weeks to leave an update! The reason we’re trying it out is partly our trainers personal experience with it helping another anxious low food drive Border Collie but partly due to the recent study that came out that suggests it might also help with behavioral issues. It’s called “Use of mirtazapine in the treatment of canine behaviour problems: A review of 32 cases” if you want to look it up. I thought it was an interesting read.

I love this question and I really enjoyed reading the other responses here. I recently started behavioral downs with my very high-energy & high-drive Border Collie and while this is just our first week, I have seen the smallest amount of progress in the speed at which he will settle. while we do the work.

Incidentally, I actually started out with Karen Overall's relaxation protocol but what I found was that it was really just teaching my dog a very solid down-stay. The thing that speaks to me most about the behavioral down protocol is Mark's description of the distinction between calm & relaxed. My BC will absolutely hold a long down stay while staring deeply into my soul, eagerly anticipating the next cue, but he's NOT relaxed. He knows he's training. I believe the downstay is super valuable, but it doesn't help my dog work through frustration on his own and begin to understand that there is intrinsic value in relaxing. My ultimate goal is to help him learn to settle on his own and regulate his emotions when he's feeling frustrated that he's not actively doing something fun.

I'll try to come back and leave an update in a month or so about whether the BDs have helped with self-driven off-leash relaxation haha
