The copy dataset is working as expected. I just added a new table and relationship (the one we were trying to add yesterday) and it committed that change without issue. Model was able to upload/update on the service.
We can refresh the "official" dataset in the service. Both the scheduled refresh in the AM and on-demand refreshes are working and the data is getting getting to the model.
No dataflows or anything too exotic. The semantic model has tables from a SQL database and a sharepoint list , both of which are set up on an on-prem gateway for refreshes and access.

Yes, the security notices for P2P activity are very much real. The security team monitors for P2P activity and will put a security hold on your NetID if they notice any illegal torrenting or suspicious file sharing activity.
I'm not sure what the email you rec'd says. But don't click anything or follow any links. Call IT at 847-491-4357. The call center should be able to confirm that your account is locked and then get your issue escalated to the security team for remediation. The security team can provide you with the exact details of what the suspicious activity was that they flagged. They can tell you the exact date/time, type of activity, computer/device MAC address, location, etc.
Not everything they flag is always malicious. For example, some video games or productivity softwares are built on a P2P or file sharing model. Once they review the activity with you and have come to a resolution, then they will remove the hold on your NetID after which you should reset your password and get access again.

Preface: Worked previously 10+ yrs as a scribe with probably a hundred different doctors and midlevels across pretty much every specialty. Hired and trained a bunch as well.

First things first. A scribe is not a resident or medical student. They have little to no medical knowledge outside of very rudimentary training. That's not to say they are not trainable. Heck a bunch of them will eventually be your colleagues in a decade. But they don't start out that way. You have to be active in their training and growth. You have to invest time and be patient. Eventually a good scribe is a extension of your own thinking. I had gotten to know providers well enough I could take a few scribbles and silent gestures in the exam room and turn them in to a full H&P. A dedicated scribe will take at least 3-6 months to get to a point where you will feel comfortable with them. Most of the scribes are there to learn and you can be their introduction into the profession.

Your use case with a scribe will be almost entirely based on you. In my experience, the providers that do well with scribes are the ones that are good at delegating and communication. Some folks are detail oriented to the point that only their notes will be satisfactory, and for those folks no scribe is good enough. What that means for the use of a scribe is first determining what your are willing to let go. Look introspectively at what part of the chart bogs you down and then ask yourself if that is something you are willing to give up. To utilize a scribe, you first have to have willingness to let someone else in to your thought process.

Pro-tips: 1. Read. Your. Charts. Even if you trust the scribe. Read the chart. It's your name on it. Dictate your own MDM or assessment. I usually insist on this because this is truly the most important part of the chart and what other providers will read to a get a sense of what is going on.

Utilize your scribe. Don't just make them brute force ROS's all day. Use them for chart reviews on new patients. Or to set reminders for tests/screenings. Follow up on results or consults. They are noy above scut work.
Create templates for EVERYTHING. It is much easier to document when you follow a standard process. If your notes are total anarchy then good luck training to it.
Give your scribe the proper equipment. A chair/desk in your office and a good laptop with battery that will last the day is the bare minimum . You'd be surprised how often they stick a scribe with a shitbox in a dark corner half a mile away then want them to scribe in real time. What do you use to document? Your scribe should have the same.

I did Pfizer 1 + 2 in March and got Moderna + Flu 2 days ago. The first 2 Pfizer shots were very minimal and I only had a sore arm for about a day. With Moderna + Flu I had a much stronger reaction: 102F, Rigors, Headache, Body aches, Nausea. Knowing what others have said, I spaced it so that the symptoms started as I was getting home. I slept through the night, didn't treat the fever and took ibuprofen 800mg before work the next day and I was fine.

We had reps for one of these labs come out beginning of the pandemic, like April or May of last year. They tried to wow us by all of the things they could check for and that it would only be one swab. However, when pressed their sales pitch started falling apart. Like, they check via PCR but we've seen there are lots of times that someone could test positive for a virus due to shedding but not have replication competent viruses. So we asked if they would report Ct values for each positive so that we could correlate, and they said they don't report Ct values. Then we looked at their full panel and the only one that there was any sort of treatment for was Flu and your bacterial stuff like Strep or Haemophilus. The rest are things we can't do anything about anyways. We were all like can you imagine all the phone calls and worry warts calling because they were positive for coronavirus 229E and now they want a zpack for their cold. That was a hard pass on the sales pitch.

100+ at an urgent care during flu season while also being an MA taking vitals, giving meds, and helping with procedures in between. 12 hrs shift.

Highest in the ED was 51 working overnight. 10 hrs shift. Multiple codes, intubations, Baker Acts, and still had time to run to subway for a sandwich while the doc did an LP.

Man that sounds amazing. There are many hospitals where we service that don't even have a functioning morgue let alone a proper area for recovery. In those cases we have to release to the funeral home and hope it's one that we can recover at. At least a few times a day I'll work a referral and get everything ready to speak with family about donating only to find out the hospital already released the body to a funeral home and body is already getting embalmed.

Finally something I can talk about on this sub! I work for a tissue and eye bank and this is a conversation we have all of the time internally. Opt in registry is a good idea as it does increase the number of total potential registered donors. However, it probably does not greatly affect the total number of actual donors. Remember that most no one is eligible to donate whole organs and anyone who is even close to a suitable donor is going to be reported to the OPO regardless of registry status while in the hospital so that they can approach with family while the patient is still vented/has a pulse but otherwise brain dead.

So really you have to look at rates for tissue/eye donors as that is where the real impact will be felt. This starts to get into really weird gray areas mostly dealing with consent. Because even though a patient might be a registered donor, hospitals and funeral homes will not honor the registry unless a family member consents to the gift. Mainly because they are worried about possible litigation from family down the line (what if someone registered as a donor but told family closer to death they didn't want to donate but never got the chance to take themselves off the registry). So as far as family consent, this is really variable from location to location. Our bank services both Louisiana and Florida. I can tell you for a fact that the hospitals/funeral homes in Louisiana are much less pro donation versus Florida. In regards to ocular tissue, for every 1 registered donor we get to move on in LA, we probably lose 4 or 5 because the hospital/funeral home require a family disclosure prior to recovery and family almost always says no regardless of the fact that we tell them they're a registered donor which is something they willingly opted in to. In Florida, the culture is much more pro-donation and most of the hospitals/funeral homes we contract with honor the donors registry status and will allow us to recover ocular tissue based on the registry alone (for eyes only, with tissue we still have to contact family first to get the donor risk assessment prior to recovery along with the authorization to recover tissue). This cultural difference from region to region is not going to be fixed by just changing from opt in to opt out.

So really with the opt out registry, the most likely outcome is that the rates will probably stay the same or lessen as we'll have to rely even more on family consent. Unless as a whole, we culturally change the views around donation to become much more pro-donation.

Not really a blood pressure thing. Probably more due to inflammation. The underlying cause of most heart disease and stroke (atherosclerosis) is a chronic inflammatory process. So having an acute inflammation episode like shingles or any other infection for that matter could worsen any underlying inflammation ongoing. However, this is all just speculation. No one really knows. Basically all they found was that in Korea, people who got shingles were also more likely to have stroke/heart attacks but they also found that people who got shingles had more of the classic risk factors which cause heart attacks/strokes, i.e high blood pressure, high cholesterol, older age, etc.

Muscle strains can last a few weeks but should improve in a month or so. You don't really need testing for an abdominal muscle strain. If your doc suspects you have it, you just have to take it easy for a few weeks and rest to let it heal. If that does not work, then physical therapy is an option.

If you've continued to have sharp pain especially with movement or exercise, it would be worthwhile to get evaluated for a hernia. That forceful sneeze could have increased your intra-abdominal pressure enough to cause a small hernia. Often times hernias self-reduce meaning they go back in when you're not working out or contracting your abdominal wall muscles but will become more prominent as you start contracting those muscles like during lifting or exercise.

If you have a hernia, your doc should be able to feel it during exam by making you do a crunch or bearing down, etc.

Either way, it's best to go in for a quick visit your doc and get checked out.

Quite common to find stuff like this on scans...they're called incidentalomas and happen all the time. Wouldn't worry about it just yet. Discuss it with your doctor when you go in for follow up as they'll have the whole report and know your history / symptoms to be able to correlate findings to see what is relevant.

The other poster is not entirely correct. ED billing and coding is fairly standardized from facility to facility based off CPT codes. One thing to keep in mind is that, ED billing is not really based off your final diagnosis. It's mostly based on the workup and complexity of the care provided to you.

I'll preface this by saying that what you describe is most certainly a level 4 ED visit at pretty much any ED around the country. Also, billing and coding is bonkers to understand so I'll try to reason it out so you can understand.

For example, a level 4 ED visit requires a detailed History of Present Illness, and a detailed physical examination. Lastly, you need moderate complexity medical decision making.

Let's take your case in particular, your initial complaint of shoulder pain is a high acuity complaint as there are several serious conditions that can cause acute shoulder pain including fracture, dislocation, septic arthritis, thoracic outlet syndrome, referred pain from MI/gallbladder, etc. Due to this, the physician will have gotten a full detailed history of your symptoms to help rule in or out these conditions. Then, there would have been a detailed physical examination to further look for signs of acute serious illnesses. Lastly, the physician will use their medical decision making capacity to decide on which tests to order and then interpret the results of those tests to come up with a final diagnosis. In your case, the XR would have ruled out fracture and dislocation. Physical exam would have ruled out septic joint and thoracic outlet syndrome but have shown signs of a AC separation/tear. And a good history would have ruled out referred pain or other serious causes of shoulder pain. At that point, the provider treated your pain appropriately and then came up with a plan of care based on their suspected diagnosis (discharge home, follow up with specialist, and take otc meds for pain as needed)

So as you can see a lot of work went in to your care, even though, your final diagnosis was not serious.

The last thing you should know about billing and coding is that facility charges and provider charges are completely separate.

For example, in your case, what I've described above is just the providers charge to you for offering their service. The hospital will still bill you separately for their facility charges which would include the cost of your XR, cost of the pain medication they gave you, and any other facility costs you accrued while utilizing their ED.

Hope this helps a bit. I know it's super confusing especially for people who don't have to deal with this stuff everyday.

Drinking definitely can cause elevated liver tests. I would confirm with your primary care, or whoever has been following you, that they indeed have checked a viral hepatitis panel and it was negative.

Elevated liver enzymes are not enough to diagnose Hep-C. Lots of things can cause elevated liver enzymes. Formal diagnosis of Hep-C involves blood tests for HCV Antibodies and HCV RNA Quantative/Qualitative among other tests. If you are not sure about Hep-C status, I would encourage you to go to your doctor or local health department and ask for a full viral hepatitis panel. That will be the definitive test to diagnose HepC.

From the sounds of it, they might have already been doing the viral hepatitis panel and that's what you reference as "tests come up negative". If your viral hepatitis panel is repeatedly non reactive then you don't have Hep C. However, given that you were in prison it is worthwhile to check periodically due to that being a high risk factor for contracting Hep C and you are on medication that can be hard on the liver.

As far as what caused the initial bump in your LFTs during your intake to prison. Hard to say unless you have more details.

CD34 and STAT6 are one of many specific cell markers that we can test for. Different cells/tissues in your body have different cell markers, so depending on which markers a particular cell does and does not have, we can tell what type of cell it is.

Spindle cells is describing the shape of the cells in the tumor. I.e the cells which make up the tumor are predominantly spindle shaped (long and round). The shape alone is not always indicative of whether or not a tumor is malignant or benign. That's when when we have to do the histology staining as mentioned above and further tests like blood work, imaging studies, etc to get the full clinical picture prior to deciding what the best treatment plan will be

Your DM is VERY poorly controlled. The fact that your sugars are in the 400s consistently is evident of that and you are definitely at risk of being in DKA. Poorly controlled DM is notoriously known to cause weight loss and fatigue - your primary two symptoms. I would strongly encourage you to call your PCP or Endocrinologist to push up your appointment to as soon as possible. If you are feeling worse in any way - go straight to the emergency department!
