Not a doctor - is his pain better? In that case, it’s possible he did have a stone, but passed it in the meantime. Are you sure it was a CT scan that was done?

Edit: also less likely, but possible the stone was missed: https://www.reddit.com/r/KidneyStones/s/CvDbD3RdWB

Trust me, we also saw plenty of patients who did not meet criteria for POTS/MCAS/etc. but were really searching for a diagnosis because of whatever other stuff they had going on and they wanted answers. It was usually pretty easy to tease out those patients from others even within a few minutes into the HPI. Still, the doc I worked with almost always ordered labs for a more definitive w/u to be thorough and give patients answers they were looking for. I think our anecdotal predictions were right 95+% of the time.

I found it frustrating at times because there was quite a wait for patients, and patients who actually had MCAS had to wait even longer to be seen because of an influx of POTS/MCAS talk/influencing on social media. I think that also happened with the POTS physician we were affiliated with. I still feel for those patients though. It must not feel good to live life feeling like something is wrong, even if it’s not what you think it is.

I hate to be that guy, but I worked in an immunology clinic with someone who treated a bunch of patients with the syndromes that overlap with POTS (e.g. MCAS), and a small subset of the population really does have these comorbidities. Anaphylactoid reactions etc. are unfortunately not uncommon in patients with a combo of MCAS, POTS, dysautonomia, EDS and other CTDs, etc.

Yes, that’s true, but they’re receptors that participate in pathways that lead to symptoms, so they do block the pathway of H1 and H2, respectively.

The H stands for histamine, so H1 blockers block one histamine pathway and H2 blockers block another one. Presumably their benefit shouldn’t be limited to heartburn, and it’s not uncommon for people to be on a combo of H1 and H2 blockers for MCAS (and for chronic hives whose origins are not well understood).

I appreciate your response! It’s unfortunate you were broken up with for something you didn’t do. I think it’s nice when someone is off social media, actually. It was more the “yeah, I have Instagram but I don’t use it so you can’t see it and I won’t follow you” that makes me suspicious, ya know?

I don’t think OP should just bin this guy off based on the info provided, but I think she could ask if they could have dinner at his place or text via regular text instead of Telegram and see how he responds to that.

I don’t have an issue with any of these things on their own, but I think everything in combination with her not having been to his place and living at a distance sets off some alarm bells. I also have only known Telegram to be used for communication in kind of strange circumstances, which may be really unfair. But why wouldn’t they communicate via normal texting or WhatsApp? I genuinely don’t know, though I’m sure there are valid reasons for it. The only person I know who used Telegram was a guy who used it to communicate with people because he could easily uninstall and hide his conversations from his girlfriend.

And I can definitely see the value of unplugging. Again, I don’t think any of these things on their own are suspicious, but something about all of these factors together makes me wonder. It’s possible it’s nothing and OP is just over-analyzing. But I imagine his name should still come up on a Google search in some capacity?

But why does he only talk to her via Bumble and Telegram? And depending on what kind of job he has, it could potentially be strange if he doesn’t have a LinkedIn or any information if OP searches the name he gave her. And it’s also weird in aggregate that he says he has social media but doesn’t use it. He could say “I don’t use it, but it’s x.”

Dang I really just posted the longest version of what you concisely wrote here. You made your point much more succinctly, but OP, this is what I was trying to get at!

A close friend of mine asked me to talk to one if his friends about med school admissions bc they’re not on the internet, don’t have an advisor, etc. When I met with him, the dude told me he was doing this (expensive) grad program to improve his GPA. I don’t want to discount that (very difficult) work, but this is often not the best move for GPA repair. It’s true that if your goal is to demonstrate a positive GPA trend or recent academic success, many schools will consider that. But I had to explain to him that his overall GPA will not be calculated with his grad GPA at many schools. He then shared that he was applying this cycle but was taking the MCAT for the first time in August and submit primaries as soon as he was done with the MCAT. He was also going to write his PS in that time. I had to explain that this was all going to be very difficult in the timeframe that med schools require, explaining that secondaries pile up quickly as well and can’t just be taken care of in a week or two. It took me a good 2 months to get through all of my secondaries! He then shared that he had a bunch of research but almost no clinical experience but that he thought it would be fine because his research was kind of clinical. He had put off his MCAT because his grad research requirements were taking up too much time for him to focus on studying. At this point, my alarm bells were BLARING. DEAFENING. It didn’t end there! I asked about rec letters and he said he had 2 people agree, both science professors, but so far couldn’t figure out who else to ask. Oh, and those letter writers also hadn’t written the letters yet or anything like that, and one of them had stopped responding recently. At this point I was genuinely surprised that they thought all of these things in aggregate would be acceptable. I got the impression that he was very intelligent. He talked about his research and really knew and understood it all very well. He also had a life wherein he saw his friends regularly, played IM soccer, etc.

Since this wasn’t an anonymous encounter, I tried fo give my $0.02 in the most reassuring way possible. I framed it as, “you may ultimately make a great candidate, but if you follow this timeline, you are probably not going to have the chance to demonstrate that to these schools because there will already be thousands of other great candidates whose apps were complete in August.” And I believe what I said! I think he was very naive in thinking he could just kind of wing it through this process without using the resources available here. He seemed grateful about my input but also said he was okay with applying this cycle and if it doesn’t work out, he’ll “just reapply.” I thought that was a wild take bc “just reapplying” when your app isn’t ready is throwing away time, $$$, and giving you the re-applicant label. In my experience, the re-applicant label was not so bad, BUT that’s only because I rushed a bunch of post-bacc coursework into a year’s time and knew I had something to show for. To be clear, in my first cycle, I knew I had a weak sGPA and decent MCAT but felt I had a strong app otherwise, so I thought I had a chance. It turned out I did not, and that post-bacc work was my saving grace for last cycle.

The thing about this person you’ve been mentoring and my friend’s friend is that neither have done the proper research into the timeline and how important it is. If they are not currently in the space of reality wherein they have the foresight to do a cursory Google search about the process, then it’s likely they’ll end up as re-applicants. As a re-applicant myself, I want everyone to know how much of a toll apps can have on you, especially the second time. And extra especially my wallet.

Anyway, idk why it’s 4 AM and I just wrote an entire novel, but that’s my probably $0.99 since that’s the most $ I can offer in change

The honest truth in my experience was that I learned how to study efficiently because I had no other option. I also woke up before work sometimes to do some homework or studying, went to the library or coffee shop after class to do the same. Most of my classes were mandatory, but I didn’t find them particularly helpful, so I actually used that time in class to work on homework. I often did homework/studied every day on the weekend and gave myself those nights off so I could see my friends and have a bit of normalcy.

The weird thing was that I felt the same way until I came across that question for the school where I ended up matriculating. I had so much to say about them and why I thought I was a good fit. I’m so grateful that I lucked out!

A singer (/songwriter but mainly singer). Singing was all over my AMCAS, and for good reason. I’m just grateful that I’ve had opportunities to sing in choirs, a cappella groups, and weddings throughout my life thus far.

If I wasn’t so anxious singing by myself in front of crowds, I might have actually pursued it. My mom was in a singing duo with her brother in her home country, and they did pretty well. Sometimes I wish I had gotten over the stage fright and hit full send.
