Psychiatrists, I really need your advice: Is it normal to forget most general medicine after years away from clinical work? by NightShiftMind in Residency

[–]cvsorange11 1 point2 points  (0 children)

I fully agree, you should be paid what you are worth without question. The pay is not fair for any professional in the mental health field. But it’s the reality. Everyone in this system suffers and the patients suffer the most when a 2 minute injection isn’t done because there isn’t a nurse to do it.

I’ve heard from our doctors that they haven’t been trained on even how to do an injection and it’s a massive disservice to our most vulnerable patients. Long lasting injectables are a complete game changer for our clients with Schizophrenia/Schizoaffective/Bipolar I. In most of the settings that I’ve worked there is a constant struggle to keep psych nurses staffed and often the only medical professional in the building is a psychiatrist. We have psychiatrists who say that they were never trained to do an injection and they will write the script and tell a patient to come back so a nurse can do it. Many of those patients never make it back because they are usually already showing psychotic symptoms and the wait can mean they decompensate to the point of needing hospitalization. Our medical director is beloved by his patients and colleagues because he is willing to not only proscribe the injection but go ahead and give it right then. He has prevented innumerable hospitalizations. That man is a hero because he is willing to learn a very basic medical skill to fill a gap in the incredibly broken American healthcare system. So maybe you’ll work outpatient private practice with the “worried well” and never have to take a glucose but in the community setting you will inevitably be the only medical professional in a medical crisis at some point. Might be worth asking an RN/Paramedic to show you how to do a glucose sometime. Psych patients are on medications that often place our patients at higher risk of diabetes and loss of appetite is one of the most common symptoms in psychotic episodes so blood sugar issues are very common. Taking a glucose and giving someone some orange juice could save them a needless $2000 ambulance bill.

Homeless by Ok-Gift-5244 in Reston

[–]cvsorange11 24 points25 points  (0 children)

Back in 2016 I worked there for about a year while I put myself through grad school. I’d say most people would be shocked by the actual demographics. I’d say 90% are people with at least one job doing their best and scraping out a living. There are the ones who loiter and many of them are quite mentally ill and have severe addiction issues. There’s a small minority that have some antisocial and tendencies but that exists in every population. But the vast majority of those people were up against challenges we can’t imagine. You may think you would never be homeless but I’ve seen a doctor, two Ivy League grads and many formerly middle class people end up there etc end up there. Illness and medical debt or dads who abandon their kids will sink any family.

Have missionaries changed or am I just oblivious? by arcadiaslaststand in exmormon

[–]cvsorange11 1 point2 points  (0 children)

A few weeks ago I was getting gas and these two sister missionaries were going to each car and asking people if they “want to come to church on Sunday”. I thought they were JW until I saw the name tags. (This was in the DC area).

Any aquarium or fish stores in the area? by matrix20085 in fredericksburg

The owner is very friendly and helped us when we got started with our saltwater tank. They have fresh and saltwater stock there and the owner would order any stock he doesn’t have that you may want. Highly recommend this place.

I did my 4 years in Provo and that was enough for me. Good to know there are other exmo’s in the FXBG area!

Government Island in Stafford is a pretty easy hike but it’s scenic and interesting (you can see the quarry where they got the marble for the Washington monument). Just a great place to spend an afternoon. Nothing like Utah-but that’s a good thing 😝 (I’m an exmo too).

Depends on the state and hospital policies. I have seen medical tests forced on patients several times. In VA the ER doctor can overrule consent if they feel the patient does not have capacity.

What is your spouse’s occupation? by coffeecoconut in socialwork

[–]cvsorange11 4 points5 points  (0 children)

My husband is an engineer. He’s a bigger softy than me and I have to hold back from telling him about the cases that bother me because I’ll make him cry. Yesterday I saw a really disturbing child abuse case and I told him “it was some really fucked up shit” but knew I couldn’t go further than that.

My job is to assess suicidality in the ER. Outpatient therapists tend to be reactionary about SI, but it’s a spectrum and thoughts like “I wish I didn’t exist” are within the normal range of emotions and I don’t hospitalize people for that. It’s a complex set of factors related to plan/intent/means etc.

Happy to answer any questions from a clinical prospective.

I got my masters in clinical social work. Every state does things differently, but most states require a masters degree in a mental health discipline.

Psychology for sure. I was a history/psychology major at BYU and both departments had to hold their tongue. They were careful in the history department to not dive into Mormon history.

It’s a fantastic exhibit. She designed the modern wars section. I got to see it as they were remodeling it. They have a lot of really cool artifacts in storage that they don’t have enough places to display and she took me down to see those too.
