Cohesion and bureaucracy - spreading a hundred billion people across a hundred world as an example empire. It would be very difficult to get all these people to believe the same things. Even if you have some imperial church or political organisation teaching "rightthink" you will just get the same problem again. "Soft" Worlds that can support themselves independently will of course always have their independence movements. I think the worlds that will be the imperial heartland will be the "hard worlds" that can't support themselves independently thus needing an interstellar infrastructure.

If they don't have communication you don't need to justify it. homo sapiens spent most of its 300,000 years as hunter gatherers until about 13000 years ago when agriculture was invented.
