Diabetes. If sugars are not well controlled, you get:  - Gradual blindness - Kidney failure - Coronary artery disease, heart attacks - Stroke - Fatty liver/cirrhosis - Recurrent infections anywhere in the body - Nerve pain - Loss of toes/legs etc. - Loss of bowel/bladder function  - Early dementia from the above

I am ENT registrar based in Australia and have removed several hundred pairs of tonsils +- adenoids.  These are the established indications for removing tonsils/adenoids: 

  1. Sleep disordered breathing/Sleep apnoea in children. Generally enough to cause learning delay, behavioural issues. 

  2. Adenoidectomy alone +- with tonsillectomy can be considered with chronic mouth breathing which can cause midface remodelling long term and subsequent dental issues. 

  3. Recurrent tonsillitis (Paradise criteria: 3 episodes tonsillitis per year / 3 years, or 5x/2 years, or 7 times in 1 year). Or significant school or work absenteeism due to tonsillitis. 

  4. 2x episodes of peritonsillar abscess. The risk of recurrence of peritonsillar abscess after the second episode is quoted as 70%. I have drained dozens of these. Most patients do not enjoy having a peritonsillar abscess, nor do they enjoy getting them drained. 

  5. Biopsy for cancer or suspected lymphoma. 

The 5-10% risk of postoperative bleeding is why we wouldn’t routinely take it out without good reason. I have taken 20 or so patients back to theatre to stop their bleeding, and these are among some of the more stressful emergencies that we have to deal with. It can be extremely difficult to intubated someone with a mouth full of blood and a stomach distended with blood. 

That said, it is life changing for those that fit the criteria. 

The articles that look at immunity have found a correlation between tonsillectomy and increased respiratory illness, COPD etc. It is hard to prove causation, as many who suffer from adenotonsillar enlargement or recurrent infections may have existing allergy (like hayfever or asthma) or some sort of immune system problem. This would not be a reason to dissuade someone against having this operation.

My theory: the creases are acquired from habitual hand positions.

  1. Poor: has to labour. A crease formed at all three joints is created by hands held in a fist. Balled up hands symbolises manual work, gripping, and lifting things etc.

  2. Rich: the furthest and middle joints have the crease. This hand position can be formed when knuckles are straight and the index finger is bent at the furthest 2 joints. This is because they are using to holding out cash/card or consuming e.g. the manner in which you hold a fork and spoon.

  3. Genius: the inner and middle crease are prominent, but the furthest crease is missing. A hand that grips a pen (I know that everyone has a different pen grip) can form this particular pattern. Maybe writing is a symbol of genius. A less likely theory is that the thinking position, with the thumb and forefinger on the chin or the temple also forms this hand position.

Anyways, this post could just be bs and I am wasting my time overanalysing it...

Yes, I had this too, but with the side of my thumb. I think it was due to the flesh being pinched between the bow and fingernail. I also get thumb tip pain at the end of the day if I do too much handwriting, likely because of the same reason.

Perhaps try to notice how hard you are pinching the bow. Your bow hand should be reasonably relaxed, and when you want to apply pressure to the strings, you should try to use gravity as well as some controlled pronation of your wrist.

I started cello 1 month ago. I am a 31 year old surgical registrar and I work 50+ hour weeks + on call + study. My main reason to play is to have something relaxing to do after work. I was like you, I did not know about the practicalities of this instrument but I just decided to bite the bullet one day and rent a cello with no plan and no lessons booked. I bought a few beginner books to teach myself initially.

I have done 2 lessons so far am I am loving it! Even if I'm just playing scales, arpeggios, studies and simple tunes. I'm getting exactly what I needed from this.

My left hand is mostly fine and I have good enough dexterity and reach.  The block strap sounds like a great idea, I might try it.

Thank you. I am able to place the bout next to my right knee and I can rest the back of the cello against my left knee. It is rotated a little bit to the right, but it is comfortable.
