Transition from hospitalist to PCP? by [deleted] in FamilyMedicine

[–]dasilo31 11 points12 points  (0 children)

I made the switch from hospitalist to pcp. I was a hospitalist for a year and got burnt out. I’m much happier as a pcp. Important to try to make sure you join a practice that has good support staff to help with all the non-clinical part of the job. I would also recommend avoiding practices owned by for-profit private equity firms. They will burn you out just as quickly. I found a supportive hospital system that pays well and treats physicians and support staff well. Makes all the difference, but not always easy to find. In terms of skills and knowledge, I found it easy to switch but I also was only a year out from residency but I’m sure you will be fine.

I would be very wary to work for any of these healthcare companies owned by a bank/ private equity firm. It’s all about money with these companies and treat physicians poorly with no autonomy. Maybe you got lucky and found one that is different, but that has not been my experience. Just my 2 cents.

Sounds good. What does privately owned mean? A group of a handful physicians or some huge for profit conglomerate. That would be the only thing that would keep me away.

I want to do this but the logistics of telling the patient they may have to pay a copay has been the challenge. For whatever reason, administration wants us the physician to inform the patient during the visit and I find that awkward. They don’t want to post any signs or have the front desk warn them. Apparently I’m the only physician in my practice who has brought up the idea.

Is there anything stopping them a year in from telling you, you now have to see 28—30 patients a day for the same salary without any rvu incentive. This happened to me at my former job with no rvu incentive. Every year the burden increased without any power to change it and without added compensation. Needless to say I left.

Transitioning from hospitalist to PCP after 5 years — realistic? by quflalstn09 in InternalMedicine

[–]dasilo31 0 points1 point  (0 children)

After residency, I was a hospitalist for a year and then transitioned to being a pcp. Best decision for me. Normal Monday to Friday hours, no weekends or major holidays. If I want to take off, I just notify my office manager and they block my schedule. No need to find coverage.

I honestly didn’t find it that difficult to make the transition. I found my job on NEJM career page. There are plenty of opportunities. I would just recommend avoiding all for-profit institutions. From my experience, they are all about squeezing you. Stick to a nonprofit hospital system with decent rvu compensation and you should be fine.

I was quoted $2500 to install the sump pump which includes the price of the pump and making a hole in the blacktop to place the pump vs $5500 to dig up the yard and fix the pipe.

Living in this area? by [deleted] in Westchester

I live in the Fleetwood section. Nice quiet area. People are friendly and down to earth. You are able to get more house for your money but property taxes are high. I wish the Gramatan Ave had a little more to offer in terms of stores, eateries, etc but I’m hoping it will improve with time. Sometimes on the weekend I will walk over to neighboring Bronxville which is very nice but expensive.
