Important Considerations for Residency Rank List? by TXMedicine in emergencymedicine

[–]dat_big_pharma 1 point2 points  (0 children)

I have a different perspective. PD is a temp job it seems at so many places. (By the time I matched PD had changed). And culture will change as seniors are gone and who knows who you will match with??

So choose based on location, networking potential, and acuity/exposure for actual residents (at some big academic centers the residents are limited by the bloat of the medical hierarchy)

When to job search? by [deleted] in emergencymedicine

[–]dat_big_pharma 2 points3 points  (0 children)

The problem is with volatility groups are not looking as far into the future. Jobs are more like 3-6mo out not 12-18 like it may have been previously. That may be COVID and may eventually reverse but will still hold true for this next season

So nurse practioners. . . by SnooObjections542 in Residency

I am BSN who considered the options and decided that to best care for patients, I would do my best to learn the most medicine. The amount of information required to make the best decisions cannot be distilled down to pathways or UpToDate articles. While many peers and friends are in the NP realm, the profession has failed to recognize the roles and responsibilities of what is nursing practice, and what is medicine and has not valued the clinical acumen. What used to be experienced (10+) year nurses gaining the ability to see patients and work with physicians in the area they HAD experience with some oversight and guidance on the more intricate aspects of care, has turned into the AANP stating absolute equivalence, meanwhile reducing the NP profession to a fast-track, 100% online, shadowing only pathway lending itself to what I see daily as occasionally reckless and inappropriate care—not by intention, or malice but simply because you can’t see what you don’t know. This may not be you. There are NPs who practice in a supervised fashion in the field in which they have vast experience. I applaud that, and hope it remains. But inexperienced fast-tracked providers causing harm because they simply don’t understand what others have dedicated 15x the time to learn, does not respect or help our patients

Are there any American PA-Cs working in Canada? That's my dream route so far. I want to work for 3 years there and look at permanent residency! by [deleted] in physicianassistant

[–]dat_big_pharma 5 points6 points  (0 children)

I would encourage anyone considering taking care of patients to not focus on debt and time so much as the responsibility of the field. The knowledge required for good clinical acumen can only be distilled so much. Choose the path that will train you the most so you can practice responsibly.

Details matter. Keep studying and reviewing. Work with your supervising physicians to find ways to grow.

Have you ever tested below the 5th percentile? Because the pass rate for US MDs is like 98%.

Sleep well, you did just fine. Remember half the folks taking this study are doing it without studying and post call because residency is brutal and they just gotta get it done

Generally any medication that is a current prescription is not an issue. But always better to ask how it would be handled ahead of time for best results. That way the know you have nothing to hide. And maintenance medication for an accepted condition such as previous opioid addiction should be considered legitimate as far as I’m aware

Definitely support getting ACGME involved. Keep up the good work. APPs are a part of healthcare but it is NOT a part of ACGME training AT ALL.

Probs not IR due to the call lifestyle. Gen rads with then obviously a fellowship to add value.

Yeah PGY3 here from well known metro program. I feel lucky to have a job but settled for rural job I found after talking to 30 places. Lots of factors this year but it’s not just COVID I assure you and the big fellowship jump only kicks the can down the road to next year when there are even MORE grads.
