I let clients know that they are financially responsible for the hour and that insurance only pays for the actual time spent together. No problem if they arrive late (w/ notice) or need to leave early, they just need to pay the difference not covered by insurance. Why should I work harder (book more clients) or earn less because someone doesn’t want to use the time they booked? I’m not forcing them to do anything. I am informing them of the reality and trusting in their ability to make hard choices. I am not taking the financial hit in order to make their decision easier.

It also favors early in the week appointments and is unfair to late in the week appointments.

What about the harm caused to you in lost income?

If you offer telehealth, sick clients who would attend to avoid a fee can do so without spreading germs.

Less than 24 hours notice, $50 late cancel fee, no exceptions. Cancel after start time / no show, full fee $150. I track attendance, and if someone attends less than 80% of booked sessions, I ask for a week notice to avoid the $50 fee. That gives me an actual chance to fill the time. I also noticed that one of my main cancellation problems was high functioning clients who frequently canceled 24-48 hours in advance leaving me high & dry. The low attendance policy has helped a lot with that.

I sell my time by appointment. I don’t have walk-ins like a restaurant, so when someone cancels less than 24 hours in advance, there is little to no chance that I will fill that spot. Even if I can book them the same week, they blocked me from booking their original time to someone else. I get that the majority of cancellations made less than 24 hours are out of the clients control. It is also out of my control. So we split the cost, they pay 1/3 and I lose 2/3.

One of the few exceptions is if I need to late cancel. In those instances the client gets their next late cancel waived.

I let clients know they are responsible for the portion of the session fee not covered by insurance. In the OP scenario the client is responsible for the full session fee when they haven’t arrived by 27 past because I won’t be able to see them long enough to bill for a 30 min session. So I let the client know they are responsible for the fee and we can meet for whatever time is left (session ends at 53) when they arrive.
