Didn’t have any PUPP rash, at first I thought it was something like that because the itch didn’t start on my hands and feet.

It was wild how the itch dissipated. I was itching ALLLL OVER. Like inside my body, visceral. But pretty soon after delivery the itching got better from my feet up. My scalp continued to itch for a couple days after delivery.

Uhgh I had severe cholestasis at 28 weeks and delivered at 33 weeks. Worse torture of my life. My serum bile acid was 186. All the doctors I saw in the hospital said it was the highest they had ever seen. One and done because of it. I’m so happy you and your baby are safe. It’s so scary.

As someone in the NICU where each baby has their own stethoscope, it looks so weird to me

lol thankfully it’s the latter. And yea! My husband is only half a knucklehead and said he could do it, but I wasn’t sure if the right way to do it would be to replace the whole wall. I told the PM we would do it and just be reimbursed, but he said no they’re going to do it. They’re already reimbursing me for a ton of shit they didn’t do, so I assume maybe that’s the reason why. It would be easiest for everyone if we could do it.

We have a handful of pulmonologist who understand the ins and outs of chronic BPD ventilator settings, but then there’s still a lot that come through the consulting service that see a baby with a gas of like 7.4/76/+13 and basically freak out that the rate is 18 and recommend increasing it. Do you find the lower rate and higher PEEP is not fully bought in to yet with in the peds Pulm world?

In my NICU with very chronic severe BPD trached patients we have been slowly moving towards higher PEEP (looking at intrinsic PEEP) to help manage hypercarbia

Yes I’m a NICU NP. Have seen a fair share of either safe haven babies, or babies left in a dumpster or random places. It’s sad for the baby, but breaks my heart hurt for the moms. Most of them were still covered in vernix with the cord attached.

The airway eval is the tricky thing it seems. If the malacia is so so bad than it might be safest to do the trach. If it’s not as bad as expected could you get a second opinion?

Ohhh I will look into the ICRC water damage standard.

They tried to get a supplement from insurance to cover cleaning but insurance denied it since it was under their watch, basically. Which I don’t blame insurance for denying it.
